Incident at 14NM ABBOTSFORD BC (CYXX)

A Swoop Inc. Boeing 737-8CT (WSW221) from London, ON (CYXU) to Abbotsford, BC (CYXX) that was cleared for the area navigation (RNAV) required navigation performance (RNP) 25 at CYXX received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) at 5000 CYXX 037/14 in Class E airspace. Traffic information was passed. The other aircraft was a Langley Flying School Inc. Piper PA-28-140 (C-GCEP) from Langley, BC (CYNJ) to Langley, BC (CYNJ).

Incident at LONDON ON (CYXU)

A Chartright Air Inc. Cessna 525C (C-FLWR/HRT202) from Montreal/St-Hubert, QC (CYHU) to London, ON (CYXU), on the area navigation (RNAV) Z 15, initiated an overshoot due to negative contact. The runway visual range (RVR) 15 was 3000. The aircraft went into a hold for approximately 15 minutes, then diverted to Hamilton, ON (CYHM). No impact on operations.

Incident at 25NM SW LONDON ON (CYXU)

An American, corporately registered Cessna 182S (N182GH) from Oakland/Troy, MI (KVLL) to Niagara Falls, NY (KIAG), transiting Canadian airspace, forgot to open its VFR flight plan (FP) with the United States (U.S.) flight service station (FSS) Leidos. The London flight information centre (FIC) filed a flight plan for the pilot when they were 25NM Southwest of London, ON (CYXU). Leidos accepted responsibility.

The London, ON (CYXU) tower was closed at 2327Z, due to short staffing. The tower closure NOTAM was issued from 2327Z to 0045Z. The Sudbury (YSB) remote aerodrome advisory service (RAAS) hours were extended to provide service cover.

Incident at LONDON ON (CYXU)

The pilot of an American, corporately registered Diamond DA 62 (N969DA) on an IFR flight from London, ON (CYXU) to Ottawa/MacDonald-Cartier, ON (CYOW) rejected their take-off from Runway 33. The aircraft taxied back to the holding bay, completed a second run-up and departed without incident at 1236Z.

Incident at LONDON ON (CYXU) (Incursion - runway - animal)

A Diamond Flight Centre Diamond DA 20-C1 (C-GDFO) was in the circuit for Runway 33 at London, ON (CYXU) and was cleared for a touch-and-go. While on the runway, C-GDFO advised the tower that 3 coyotes were crossing the runway ahead of them and that they were coming to a stop. No impact to operations.

Incident at LONDON ON (CYXU)

A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GDFO) from London, ON (CYXU) to London, ON (CYXU) departed from Runway 27 at 1637Z for a local VFR flight to the Southwest of CYXU, and was issued a left turn out. A Diamond Flight Centre London Inc. Diamond DA 20-C1 (C-GDFB) from London, ON (CYXU) to London, ON (CYXU) departed from Runway 27 at 1638Z for a local VFR flight to the Northwest of CYXU and was instructed to maintain the runway heading due to traffic. At 1639Z, C-GDFO was observed in a right turn out and was instructed to fly heading 270. C-GDFO was subsequently observed in a left turn, Southbound through the departure path of C-GDFB, placing them in a position to cut off C-GDFB. C-GDFB was passed traffic. The pilot of C-GDFB reported that C-GDFO was in sight and turned on course behind them. Minimal impact on operations. (Duplicate AOR-266912-V2 was deleted after information was merged)

Incident at LONDON ON (CYXU)

The intersections of Runway 09/27 and Runway 15/33 were NOTAMd closed when a Midland Instruments Piper PA-34-200T (C-GAPD) from London, ON (CYXU) to Toronto/Billy Bishop, ON (CYTZ) requested taxi clearance from Apron 01 for a VFR flight to CYTZ. C-GAPD was instructed to taxi to Runway 15 at intersection Delta, for departure via Taxiways B, G and D, but instead C-GAPD taxied B, G and across Runway 09/27 without authorization. No impact.

Incident at KINGSTON ON (CYGK)

The London, ON (CYXU) flight information centre (FIC) received an arrival message from the Kingston, ON (CYGK) flight service station (FSS), but there was no flight plan on file for a 1991951 Alberta Ltd. Piper PA-32R-301 (C-GYFA) from Orillia Rama, ON (CNJ4) to Kingston, ON (CYGK). The logs were checked and there was a flight plan from CYGK to CNJ4 earlier in the day, but no return leg. The pilot was contacted and they advised that they filed both flight plans on flightplan.com. London advised the pilot that they did not have alerting for the leg from CNJ4 to CYGK. No operational impact. (Dupilcate AOR-266887-V3 was deleted after the information was merged)

Incident at 58NM LONDON ON (CYXU)

The Cleveland, OH (CLE) centre advised that there was a Sunwest Aviation Ltd. Learjet 35 (CNK633) from Calgary, AB (CYYC) to Hamilton, ON (CYHM) that was "no radio" (NORDO) and transiting their airspace while enroute to CYHM. At 2102Z, the CE controller accepted a radar handoff of the NORDO flight. Communication was established at 2107Z. The North American Aeropsace Defence Command (NORAD) was advised.

Incident at 13NM LONDON ON (CYXU)

A 2395173 Ontario Limited Beech B300 (C-GRGF) from Parry Sound, ON (CNK4) to London, ON (CYXU) was level at 5,000 feet and cleared for an approach, when it reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed due to VFR traffic crossing in front at 4,500 feet. Traffic had been passed and was reported as observed on TCAS prior to the RA.

Accident at 5NM W HORNEPAYNE MUNI ON (CYHN) (Aerodrome - other)

The pilot of a privately registered Cessna A185E called the London, ON (CYXU) flight information centre (FIC) to advise that their motor stopped while on approach and they landed in the trees at Government Lake, ON just west of Hornepayne, ON (CYHN). The VFR flight plan was closed. No assistance was required. No injuries. The shift manager was notified. The NAV Canada Manager (NCM) was notified.
Update TSB Report #A20O0075: A privately owned, float equipped Cessna A185E aircraft, with one soul on board, was on a VFR flight from Riviere de Milles Isles, approximately 12 SM North of Montréal/Pierre Elliott Trudeau International Airport (CYUL), QC, to the Geraldton/Hutchison Lake Seaplane Base (CNE6), ON. Upon arrival at CNE6, the pilot was advised that no fuel was available and as a result flew to Government Lake, located 5 nautical miles West of the Hornepayne Municipal Airport (CYHN), ON. During the approach to Government Lake, the engine failed (Continental IO-520) and the aircraft impacted the trees approximately 200 feet prior to the shore line. The pilot was not injured but the aircraft was substantially damaged.

A privately registered Beech B60 flying VFR from London, ON (CYXU) to London, ON (CYXU) landed Runway 09 and exited via Taxiway Alpha. The B60 was instructed to taxi via Alpha and hold short of Runway 15, which the pilot read back. A privately registered Aero Commander 112 flying VFR from London, ON (CYXU) to Whitehorse/Erik Nielsen, (CYXY) on a simulated approach for Runway 15 was cleared to land Runway 15. The BE60 was observed entering Runway 15 at Alpha, with the AC112 short final for Runway 15. The tower instructed AC112 to pull up and go around. The BE60 stopped on Runway 15 at alpha.

BOW103 advised they were stopping to give way to C-GDFB at the Taxiway G/A intersection.

A Smiths Falls Flying Club Inc. Cessna 172M (C-GPTW) from Smiths Falls-Montague (Russ Beach), ON (CYSH) to London, ON (CYXU) entered the Toronto/Lester B. Pearson, ON (CYYZ) terminal Class C airspace without a clearance. Communication was eventually established with the area control centre (ACC). C-GPTW subsequently entered the Toronto/Billy Bishop, ON (CYTZ) control zone without a clearance. Communication was not established until closer to the CN Tower. No operational impact.

Incident at LONDON ON (CYXU)

A Jazz de Havilland DHC-8-301 (C-GTAQ/JZA8646) from London, ON (CYXU) to Toronto/Lester B. Pearson, ON (CYYZ) was cleared for take-off from Runway 33, and was instructed to contact the Toronto Centre once airborne. While C-GTAQ was rolling, a 7506406 Canada Inc. Agusta AW139 (LF3) on a MEDEVAC flight from London, ON (CYXU) requested departure off of Apron II, northbound. LF3 was instructed to take-off northwest-bound, with a restriction of "no further East than Golf" and a wake turbulence cautionary for JZA8646. LF3 departed Apron II, and was instructed to proceed on course once JZA8646 was in sight. LF3 reported that JZA8646 was in sight and passed behind and below JZA8646, northbound. JZA8646 then contacted the tower about 5 miles northeast and reported receiving a resolution advisory (RA) on departure. No operational impact.

A St. Thomas Flight Centre Cessna 172M (C-GXMF), on a flight to St. Thomas, ON (CYQS), entered the London, ON (CYXU) control zone without authorization and was observed nearing two VFR flights, a privately registered Cessna 150M from St. Thomas, ON (CYQS) to London, ON (CYXU), and a privately registered, amateur-built Vans RV-14A on a flight to London, ON (CYXU), that were within 1-2 miles of C-GXMF. Traffic was passed to other traffic. No other impact on operations.

Incident at LONDON ON (CYXU)

At London, ON (CYXU), an emergency locator transmitter (ELT) signal was heard on frequencies 121.5 and 243.0. The ELT began around 1740Z and continued until the Joint Rescue Coordination Centre (JRCC) Trenton located a privately registered Piper PA-28-180 in the hangar at CYXU around 2300Z, which was determined to be the source.

An inbound Jazz de Havilland DHC-8-301 (C-GTAT/JZA8643) from Toronto/Lester B. Pearson, ON (CYYZ) to London, ON (CYXU) reported a possible bird strike upon landing on Runway 27. The runway was inspected, and nothing was found. Minor impact: one VFR aircraft was delayed while Runway 09 was inspected.

An ITPS (Canada) Ltd. Aero L-29 Delfin (C-FLVB/ST45) from London, ON (CYXU) to London, ON (CYXU) and an ITPS (Canada) Ltd. Aero L-29 Delfin (C-FXZI/ST46) from London, ON (CYXU) to London, ON (CYXU) were instructed to taxi on Taxiway A for departure from Runway 33, expect the backtrack from A, and contact the tower while holding short of Runway 33. ST45 contacted the tower while on Taxiway A, and as they were broadcasting the lead aircraft, ST45, was observed crossing the hold short line for Runway 33, coming to a stop approximately 10 feet past the line. No impact to operations.

An American corporately registered Diamond DA 42-NG (N42LA) from London, ON (CYXU) to Detroit, MI (KDET) departed Runway 27 and requested a return to CYXU citing an unsafe gear indication. The aircraft performed a low approach over Runway 33 to allow the tower to examine the landing gear, and landed safely on Runway 27 at 2241Z. No assistance requested, but Aircraft Rescue and Fire Fighting (ARFF) was standing by. No impact to operations.

The pilot of a privately registered Cessna 172L from London, ON (CYXU) to London, ON (CYXU) called the tower on Taxiway D, ready for departure Runway 15. At the time, a deer was observed by the tower to be crossing Runway 15 south of Taxiway H. The aircraft was offered, and accepted, a Runway 33 departure from Taxiway A to avoid the deer. Significant deer activity is noted at the CYXU.

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3516) from London, ON (CYXU) to Toronto/Lester B. Pearson, ON (CYYZ) declared PAN PAN while inbound due to a flap issue. WEN3516 landed on Runway 06L without further incident (WOFI) at 1933Z.
Update TSB Report #A20O0062: WestJet Encore flight WEN3516, a Dehavilland Dash 8 Q-400 (C-GWJK) aircraft, on route from London, ON (CYXU) to Toronto/LBPIA (CYYZ), ON, declared a PAN PAN due to flap issues. The aircraft continued to its destination where it landed without further incident. Maintenance found the Flap lever trigger to be sticking. Flap lever selector trigger mechanism was lubricated and flap functional test carried out, checked serviceable. The aircraft was returned to service.

Incident at LONDON ON (CYXU)

Aviation Incident Report #16655: After take-off, following the retraction of take-off flap, an ITPS (Canada) Ltd. Aero L-39C (C-GFNO) from London, ON (CYXU) to London, ON (CYXU) experienced excessive nose-up forces that made the aircraft difficult to control. The elevator trim did not seem to work. The pilot slowed the aircraft and returned to CYXU. C-GFNO landed without incidence. Maintenance (MTC) inspected the aircraft without being able to identify a specific fault.

An unknown aircraft on the ground at London, ON (CYXU) squawked 7700 for approximately three minutes. Four CF-18 aircraft, including a Government of Canada, Department of National Defence McDonnell Douglas CF-18 Hornet (QUILL01) from London, ON (CYXU) to Bagotville, QC (CYBG) and a Government of Canada, Department of National Defence McDonnell Douglas CF-18 Hornet (ALUT26) from London, ON (CYXU) to Trenton, ON (CYTR), were starting up at the time on Apron 2 and were queried, but they responded that they had not squawked the code. No other aircraft were observed to be active on the aprons, and none responded to the query. No impact to operations.

The London, ON (CYXU) Flight Information Centre (FIC) received a call from the Hamilton, ON (CYHM) tower to close a VFR flight plan on an American, corporately registered Aerospatiale AS350 B3 (N125TG) on a transborder flight from West Mifflin, PA (KAGC) to Hamilton, ON (CYHM). The CYXU FIC had no information on the aircraft. Upon further investigation, FlightAware indicated that the helicopter had departed KAGC northbound to Hamilton. Leidos confirmed they also had no information on this flight.

Incident at 4NM NW LONDON ON (CYXU)

A privately registered Cessna 172N from Kitchener/Waterloo, ON (CYKF) to Kitchener/Waterloo, ON (CYKF) reported hearing an emergency locator transmitter (ELT) approximately 4NM Northwest (NW) of London, ON (CYXU). Signal was first heard 3NM NW intermittently during climb to 3000 ft. The signal was steady for approximately 2 minutes and was last heard 8NM NW of the field at 3000 ft. No other aircraft reported the signal. Signal was not received in the tower or the flight information centre (FIC). No impact on operations.

An American, corporately registered Diamond DA 42 NG (N421BL) from London, ON (CYXU) to Raleigh-Durham, NC (KRDU) was cleared for takeoff from Runway 33 and was instructed to contact the Toronto Centre when airborne. On departure, the aircraft was observed in an abnormal yaw, contacted the Tower and requested to return for landing without stating a reason. The aircraft was cleared for a right downwind for Runway 33 and landed normally at 1739Z. No emergency declared, no impact to operations.

Air traffic control (ATC) cleared a WestJet Encore de Havilland DHC-8-402 (C-FTEN/WEN3516) from London, ON (CYXU) to Toronto/Lester B. Pearson Intl, ON (CYYZ) to descend to 8000ft. WEN3516 read back clearance to 8000ft. WEN3516 started to descend to 6000ft. No operational impact.
UPDATE AOR-263516-V3: WEN3516 read back a clearance that had been issued to a Morningstar Air Express Cessna 208B (C-FEXO/MAL7056) from Sudbury, ON (CYSB) to Toronto/Lester B. Pearson, ON (CYYZ) to descend to 6000ft. Separation was not assured between WEN3516 and a Delta Air Lines Bombardier CL600-2424 (N297PQ/EDV5024) from Toronto/Lester B. Pearson, ON (CYYZ) to Detroit/Wayne County, MI (KDTW).

Incident at LONDON ON (CYXU)

The Joint Rescue Coordination Centre (JRCC) Trenton (YTR) called at 1500Z to inquire about a 406 emergency locator transmitter (ELT) signal in the London area. Nothing was heard on frequency until 1530Z, with a strong signal on both 121.5 and 243.0 for 1 minute, then a continuous blip signal on 243.0 for several hours. All on-field agencies were called to check fleet. Nothing found by 1800Z, but a weak blip signal continues on 243.0. The Toronto (YYZ) shift manager and JRCC Trenton were notified.

At approximately 1230Z, the Joint Rescue Coordination Centre (JRCC) Trenton called to advise of an Emergency Locator Transmitter (ELT) signal, unregistered to any aircraft, that was activated in the vicinity of London, ON (CYXU). The ELT signal was audible by the London Tower on backup (TIL) radio only, and was a strong signal. The signal stopped at approximately 1245Z. Multiple facilities on the field were contacted by the London Tower. JRCC Trenton traced the ELT to the Diamond Factory.

Incident at 20NM SSE LONDON ON (CYXU)

A Flightexec Gulfstream 200 (C-GRST/FEX200) from London, ON (CYXU) to Liberia, Costa Rica (MRLB) declared an emergency in Cleveland Air Route Traffic Control Center (ZOB) airspace due to an indication issue and requested a return to CYXU. FEX200 subsequently cancelled the emergency because the indication cleared and returned to CYXU after a fuel dump. FEX200 landed CYXU at 1526Z without incident. No impact to operations.

UPDATE TSB Record #A20O0035: Flightexec flight QG5MLI, a Gulfstream G-200 (C-GRST), departed London (CYXU), ON and was climbing through FL230 when the "Master Caution" illuminated and CAS displayed "R BLEED AIR". The flight crew notified ATC and levelled off, after which the QRH checklist was completed. The flight crew decided to not shut down the affected engine; an emergency was declared and the flight was cleared back to CYXU. The flight crew informed ATC that they had to dump fuel and were told to dump while over the Lake Erie area. During the fuel dump process the "R BLEED AIR" indication went away. Subsequently the flight crew cancelled the emergency and returned to CYXU, and landed without further incident.

The London, ON (CYXU) Flight Information Centre (FIC) received a VFR proposed flight plan for a Seneca College of Applied Arts and Technology Cessna 172S (C-FSEV) from Burlington, VT (KBTV) to Peterborough, ON (CYPQ) at 1706Z, with a 1730Z estimated time of departure (ETD). The FIC never received a departure from Leidos. The aircraft landed safely in CYPQ at 2155Z.
