Incident at 6045N / 07269W

At 1915Z, a Cargojet Airways Ltd. Boeing 767-39H (C-FGSJ/CJT462) from Winnipeg/Richardson, MB (CYWG) to Iqaluit, NU (CYFB) was cleared for DCT YFB. At 1934Z, an automatic dependent surveillance – contract (ADS-C) report indicated that the flight was not following the cleared route even though the pilot confirmed that he was following DCT YFB. The flight was seen 2 NM off course and straying away from the cleared route. No impact on operations.

Incident at Kelowna

A Pacific Coast Jet Charter Inc. Cessna 525B (N525CR/PXT525) from Kelowna, BC (CYLW) to Seattle/Boeing Field, WA (KBFI) was given taxi instructions that included a hold short of Runway 16. PXT525 read back the hold short but, upon taxiing, crossed over the hold line and incurred on Runway 16 with a WestJet Encore Ltd. de Havilland DHC-8-402 (C-GWFE/WEN3181) from Winnipeg/Richardson, MB (CYWG) to Kelowna, BC (CYLW) on a 5 mile final. The arriving WEN3181 was instructed to reduce as safely able and continue approach as an aircraft had incurred and was clearing the runway. The arriving WEN3181 was given their landing clearance at 3NM final and landed safely. No other operational impact.

A Flair Airlines Boeing 737-4Q8 (C-FLHJ/FLE134) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Toronto/Lester B. Pearson, ON (CYYZ) declared a PAN PAN emergency when 4NM final for Runway 24R at CYYZ due to a hydraulic issue. They elected to continue the approach with no equipment required. The aircraft landed safely at 2037Z and taxied to the gate without further incident.
UPDATE TSB Report #A20O0038: Flair Airlines flight FLE134, a Boeing 737-400 (C-FLHJ) was on final approach to Toronto /LBPIA (CYYZ) Runway 24R, when a master caution illuminated and a number of indications were lost, including: hydraulic, engine, electrical, flight control, and pneumatic. The flight crew declared a PAN PAN and determined that it was only indication problems. The aircraft landed on Runway 24R without further incident, taxied clear of the runway, and had the aircraft inspected for hydraulic leaks.

After departure, a Sky Regional Airlines Embraer ERJ 170-200 SU (C-FEJL/SKV7557) from Toronto/Lester B. Pearson, ON (CYYZ) to Winnipeg/James Armstrong Richardson, MB (CYWG) indicated they had an issue with their nose gear door, and would like to return to CYYZ, after burning off fuel to reduce landing weight. TSB Report #A20C0019: C-GJVO, a Fairchild SA227-DC aircraft operated by Perimeter Aviation as Bearskin Airlines flight 312, departed the Winnipeg/James Armstrong Richardson Intl. (CYWG), MB. for Red Lake (CYRL), ON with a crew of 2 and 5 passengers on board. Shortly after takeoff, the crew received a fire warning indication on the right engine. The crew decided to shut down the engine and return to CYWG. An emergency was declared and the crew then completed a successful single engine landing. An inspection of the aircraft by the operators maintenance revealed a faulty fire probe in the right engine bay area.

TSB Report #A20W0024: C-GXWJ, a WestJet Boeing 737-600 operating as WJA493, was conducting a flight from Winnipeg (CYWG), MB to Calgary (CYYC), AB. During descent, the flight experienced a cabin depressurization which resulted in the cabin masks deploying and the cabin altitude warning activating. The flight crew increased their rate of descent and advised ATC. No PAN or MAYDAY was declared. The aircraft landed safely a 1300 MDT. Update: Maintenance replaced the outflow valve and oxygen generators and the aircraft was returned to service.

Incident at CALGARY INTL AB (CYYC)

Aviation Incident Report #16567: A call was received from the Calgary, AB (CYYC) airport that local police were removing a passenger from a WestJet Boeing 737-7CT (C-GWBT/WJA202) on a flight from Calgary Intl, AB (CYYC) to Winnipeg/James Armstrong Richardson Intl, MB (CYWG). This passenger had been arrested and released in Edmonton, AB (CYEG) earlier that day after an incident on another airline aircraft and was therefore denied boarding in CYEG. After the arrest, the passenger then bought a ticket on WestJet for flight WJA202 and departed to CYYC before anyone realized it. WestJet Corporate Security notified CYYC staff that the passenger was to be denied boarding. Local police were called to assist as the passenger was already on the plane. Passenger was removed from the flight with no incident and minimal delay to the flight.

Aviation Incident Report # 16572: A guest was witnessed by crew members of a WestJet flight leaving their seat and going into the aft lavatory. Upon exiting the lavatory, an odor of cigarette smoke was present which was confirmed by crew. Crew confirmed that they had a conversation with the guest who admitted to smoking. The Royal Canadian Mounted Police (RCMP) met the aircraft upon arrival at gate, boarded the aircraft and escorted the guest off.

A vehicle doing an inspection of the runway did not respond to radio calls to exit when a Keewatin Air Beech B200 (C-FGWR/KEW212) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Churchill, MB (CYYQ) was on approach. The aircraft elected to fly a low and over. The vehicle exited the runway and called off, indicating it had heard no calls. A Keewatin Air Beech B200 (C-FKEX/KEW217) from Churchill, MB (CYYQ) to Winnipeg/James Armstrong Richardson, MB (CYWG) delayed departure by about 2 minutes while waiting for the initial aircraft to make its second approach and land.

Once emergency vehicles were in position, FAB152 landed at 0005Z without incident and stopped on the runway for a visual inspection by emergency services before taxiing to main apron. The aircraft deviated 40 degrees off course while passing through the ILS 31. The controller observed and corrected immediately with vectors to 36.

Incident at 20NM E EDMONTON INTL AB (CYEG)

A Sunwest Aviation Learjet 35A (C-GTJL/CNK633) MEDEVAC from Winnipeg/James Armstrong Richardson, MB (CYWG) to Edmonton, AB (CYEG) was 20NM East of CYEG and declared Mayday due to an engine failure. Emergency services were arranged. CNK633 landed safely on Runway 30 at 0146Z. A WestJet Encore de Havilland DHC-8-402 (WEN3132) from Fort McMurray, AB (CYMM) to Edmonton, AB (CYEG) was delayed to allow CNK633 and emergency vehicles to clear the runway.
UPDATE TSB #A20W0023: The Sunwest Aviation Learjet 35, C-GTJL, operating as CNK633 on a flight from Winnipeg (CYWG), MB to Edmonton (CYEG), AB was on a descent through 12 000 feet when the left hand engine (TFE-731-2-2B) started to lose power. The throttle was advanced to try and regain power but the engine power continued to decrease. As the engine continued to lose power, the associated low oil press light came on and triggered the master warning. The crew declared a Mayday and asked for delaying vectors so they could run the associated checklists. Once the engine was secured, the flight crew told ATC they could continue inbound for a single-engine approach and landing. ARFF was requested to stand by for the landing. The landing and taxi to the FBO happened without further incident. Update: The engine was inspected and motored with no faults found. The digital electronic engine control (DEEC) had recorded the engine shutdown but there were no other indications of a fault. The aircraft had been over-wing refueled in Winnipeg (CYWG) MB by two trucks; one on each wing. The fuel request showed that additive was requested and both trucks were capable of providing the same. The aircraft was refueled after the maintenance checks in CYEG and engine test runs were undertaken with no faults found. The aircraft was then flown under a ferry permit back to Calgary (CYYC), AB for further investigation.

PAG1751 returned to the terminal control unit (TCU) for vectors and landed on Runway 31 safely at 1805Z. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Upon gear down selection, the Left-Hand (LH) Main landing gear was not indicating down and locked. Light tested serviceable. Rectification as follows: Main landing gear assemblies cleaned and lubricated due to build up of fine gravel debris. Functional check of normal and emergency system extension and retraction carried out serviceable.

A Delta Air Lines Airbus A319 (DAL1319) from Calgary, AB (CYYC) to Minneapolis-St. Paul, MN (KMSP), departing CYYC and climbing to 9000 feet, received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) on a Jazz Bombardier CL-600-2D24 (JZA8333) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Calgary, AB (CYYC) descending to 10000 feet on the BIRKO 4 ARR. Traffic was passed. DAL1319 levelled at 8700 feet.

A Cargojet Airways Boeing 767-323 (C-GAJG/CJT566) from Calgary, AB (CYYC) to Winnipeg/James Armstrong Richardson, MB (CYWG) was pulled by the tower due to traffic in position and previous arrival still on the runway. The aircraft went back to arrival for another approach and landed safe. No other impact to operations.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

The arrival controller informed the tower that a Morningstar Air Express Inc. Boeing 757-2B7 (C-FMAI/MAL7050) from Toronto/Lester B. Pearson, ON (CYYZ) to Winnipeg/James Armstrong Richardson, MB (CYWG) had a flap issue and was requesting Aircraft Rescue and Fire Fighting (ARFF) on standby. Aircraft landed safely at 0742Z. MAL7050 stayed on the runway for a few minutes while ARFF inspected the aircraft. No other impact to operations.

Incident at 10NM BRANDON MB VOR/DME

A Swoop Boeing 737 600 (WSW402) from Abbotsford, BC (CYXX) to Winnipeg/James Armstrong Richardson, MB (CYWG) was observed on radar turning off course. The controller provided vectors until the aircraft advised it was able to continue on the BEFAN 2 arrival. No impact to operations.
