Incident at BAGOTVILLE QC (CYBG)

Aviation Incident Report #16540: At Bagotville, QC (CYBG), a runway incursion was committed by a foreman at the intersection of Runway 18/36 and Runway 11/29 while a Jazz de Havilland DHC-8-301 (C-GETA/JZA8684) from Montreal/Trudeau, QC (CYUL) to Bagotville, QC (CYBG) was landing on Runway 36. Foreman was instructed to proceed on the airfield, holding short of Runway 36, and had read back the restriction. A few minutes later, the foreman was on Runway 29, driving towards Runway 36 faster than expected, so the ground controller reiterated to the foreman to hold short of Runway 36. This was followed by the instruction to stop immediately when the vehicle was seen crossing the hold short line. The vehicle was then immobilized at the center of the main intersection when the aircraft touched down. Ground control issued instructions to the vehicle to vacate the runway without delay, while the tower controller told the pilot about the unauthorized vehicle at the main intersection. The pilot had a visual on the vehicle and, after touching down, decided to pull up. The aircraft ended up conducting an overshoot less than a 100 above the vehicle when it was exiting the runway. The aircraft stayed on the tower frequency and came back for a landing on the active runway (Runway 29) without further incident.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Aviation Incident Report #16539: Upon pushback from the gate, the pilot of a Royal Jordanian Airlines Boeing 787-8 (JYBAH/RJA90G) from Montreal/Trudeau, QC (CYUL) to Detroit/Wayne County, MI (KDTW) specified that they did not need to proceed to the de-icing facilities, and the aircraft was cleared to taxi, and instructed to hold short of Runway 06R. As the aircraft passed Taxiway AM, the pilots of an Air Canada Airbus A320 (ACA321) queried as to whether RJA90G had been de-iced. The controller mentionned that RJA90G did not request de-icing, to which ACA321 replied that this decision should be challenged. RJA90G had already been handed off to the tower at this point, so the information was relayed to the tower via the hotline. The information was then relayed to the pilot of RJA90G. Regardless, RJA90G departed without de-icing. Approximately 1 minute after the call from ACA321, the pilots of an Air Canada Boeing 787-8 (ACA417) that was waiting on the north ramp as RJA90G taxied past mentionned that one of their passengers reported that RJA90Gs wings were covered with snow. In the hour preceding RJA90Gs departure, all flights departing from the apron required de-icing services.

Incident at BAGOTVILLE QC (CYBG)

Aviation Incident Report #16597: At 1106Z, a Hydro-Quebec de Havilland DHC-8-402 (C-GHQL/HYD162) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Bagotville, QC (CYBG) conducted an overshoot while on final approach. The pilot requested a 360 degree turn and, having a visual with the airfield, requested to proceed with a visual approach, which cannot be authorized by a flight adviser, so the aircraft was sent back to the terminal controller. The aircraft did not answer to the frequency change instruction and started their 360 degree turn. This led HYD162 to be facing a Provincial Airlines de Havilland DHC-8-315 (C-FPAE/PVL3043) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Bagotville, QC (CYBG), which was established on final. The flight adviser (FA) gave traffic information to both pilots and then HYD162 turned on a south heading and contacted the terminal. Separation at this moment was 2.2NM between the aircraft, with less than 1000ft vertical separation between them. The terminal controller then brought the aircraft back on a visual approach with no more issues. No impact on operations.

Unexpected failure of the Extended Computer Display System (EXCDS) flight data element (FDE) at variable durations for all Flight Information Region (FIR) and Area Control Centre (ACC) sites between 1103Z and 1245Z. For the Montreal/Pierre Elliott Trudeau, QC (CYUL) tower (TWR), a simultaneous NAV CANADA Auxiliary Radar Display System (NARDS) failure occurred between 1125Z and 1130Z. Impact: Return to the use of paper sheets for the duration of the outage.

A Government of Canada, Department of National Defence Lockheed C-130 (Hercules) (CNUK11) from Trenton, ON (CYTR) to Trenton, ON (CYTR) received and followed a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) while in flight. A Cargair Ltee. Cessna 172N (C-GVXM) from Montréal/St-Hubert, QC (CYHU) to Montréal/St-Hubert, QC (CYHU) was at 500 feet below. Traffic was exchanged. No loss of separation and no other impact on operations.

At Kuujjuarapik, QC (CYGW), the transmitter (TX) for remote communications outlet (RCO) 122.2 was not working, but the receiver (RX) was functioning. The Technical Operations Coordinator (TOC) was advised. 4 aircraft were inbound: an Air Inuit Ltd. de Havilland DHC-8-314 (C-GRAI/AIE4802) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Kuujjuarapik, QC (CYGW); an Air Inuit Ltd. de Havilland DHC-6-300 (C-GNDO/AIE648) on a flight to Kuujjuarapik, QC (CYGW); a Voyageur Airways de Havilland DHC-8-102 (C-GOND/VAL9886) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Kuujjuarapik, QC (CYGW); an Air Creebec Inc. de Havilland DHC-8-314 (C-GZEW/CRQ235) from Waskaganish, QC (CYKQ) to Kuujjuarapik, QC (CYGW). All on the ground without incident. Other impacts unknown. CYGW RCO 122.2 serviceable (svcbl) at 1542Z.

On the approach, an Air Inuit de Havilland DHC-8-314 (C-GIAB/AIE4208) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Kuujjuarapik, QC (CYGW) performed a missed approach and declared an emergency due to hydraulic problems, and requested to be redirected to La Grande Riviere, QC (CYGL). AIE4802 landed safely at 1552Z.
UPDATE TSB Report #A20Q0046: C-GIAB, a De Havilland DHC-8-314 operated by Air Inuit, was conducting flight AIE4802 under instrument flight rules, from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Kuujjuarapik, QC (CYGW) with 3 crew members and 15 passengers on board. While the aircraft was 8 nm on final approach to CYGW, when selecting the landing gear in the lowered position, the amount of hydraulic fluid in system number 2 dropped to 1 pint. The crew made an ascent and an emergency (PAN PAN) was declared. The crew diverted to Grande Rivière, QC (CYGL) with the landing gear in the lowered position. The aircraft landed safely at 1552Z.

Signal became intermittent at 13 000 ft and stopped at 8 000 ft. The Joint Rescue Coordination Centre (JRCC) Trenton (TR) was advised. Various aircraft in the vicinity reported the ELT. Last positive report was received at 2135Z.

TSB Report #A20Q0043: C-GGOF, a Jazz Aviation LP Dash-8 Q400 (JZA8732), was on an instrument flight rules (IFR) flight from Montreal Pierre Elliott Trudeau, QC (CYUL) to Quebec Jean Lesage, QC (CYQB) with 65 passengers and 4 crew members. During the flight, a member of the cabin crew became physically incapacitated. The second member of the cabin crew assigned a passenger to watch the emergency exits at the back of the aircraft for the landing. The aircraft landed at its destination without incident. No one was injured, and there was no damage to the aircraft.

While the aircraft was taxiing on the ground to position itself near the passenger boarding and unloading gate, the right engine (#2) turned off on its own. The aircraft was taken out of service and the engine #2 electrical control unit (ECU) was replaced.

Smoke was observed from the left engine of a Sunwing Airlines Boeing 737-86J (C-GOWG/SWG9012) on departure from Montreal/Pierre Elliott Trudeau, QC (CYUL) for Hamilton, ON (CYHM). The pilot was advised by the terminal control unit (TCU). Twenty minutes later, a fuel spill was observed on Taxiway A up to Runway 24L. An inspection confirmed that the spill was from SWG9012 at the gate. The Montreal shift manager (GDQ) was notified. No impact to operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

TSB Report #A20Q0024: C-GABO, a Jazz Aviation LP Dash-8-300 (DH8C) (JZA8970), was on an IFR flight from Ottawa/Macdonald-Cartier, ON (CYOW) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 48 passengers and 3 crew members on board. After the landing and while the aircraft was taxiing to the terminal, a passenger alerted the crew to flames coming out from the right main gear. The crew brought the aircraft to a stop and requested the intervention of Aircraft Rescue and Fire Fighting (ARFF) services. The fire quickly went out and the aircraft was towed away to the passenger bridge. When the aircrafts right main gear was checked, a breakage was found in two bearings in the inboard wheel.

A Jazz de Havilland DHC-8-402 (C-GGFP/JZA7977) on a flight from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Toronto/Billy Bishop, ON (CYTZ) declared an emergency due to a gear indication and elected to return to CYUL. JZA7977 cancelled the emergency at 0006Z and landed safely at 0020Z. No impact on operations.
TSB Report Update #A20Q0041: An Air Canada Jazz Dash 8-400 (C-GGFP/JZA7977) was on an IFR flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Toronto City/Billy Bishop, ON (CYTZ). During the initial climb during departure from CYUL and after the retract gear function was selected, the gear first went into the retracted position but then went out again shortly afterwards, even though the extension function had not been selected. The crew carried out the manual gear extension procedure. An emergency was declared; the crew asked to return to CYUL, where the aircraft landed without incident. The landing gear selector panel was changed and the aircraft was returned to service.

Incident at 12NM MANIWAKI QC (YMW) NDB

A WestJet Boeing 737-7CT (C-FWSF/WJA214) from Calgary, AB (CYYC) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was cleared to FL310 with a correct read back. WJA214 was informed of traffic at FL300. WJA214 reported having the traffic in sight. At FL314, with a descent rate of around 2900/minute, WJA214 advised air traffic control (ATC) of a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed a few hundred feet. The other aircraft in the vicinity was a Japan Airlines Boeing 787-9 (JA865J/JAL7) from Boston, MA (KBOS) to Tokyo/Narita Intl, Japan (RJAA). After crossing traffic, WJA214 was observed descending below FL310. No impact.

Incident at VAL-D'OR QC (CYVO) (ATM - inaccurate aeronautical information)

A Jazz de Havilland DHC-8-300 (JZA8750) from Val-dOr, QC (CYVO) to Montreal/Pierre Elliott Trudeau, QC (CYUL) at 60 NM SE FL200 reported that it had lost contact (CTC) with Montreal on frequency 125.9. Montreal was notified. The technical operations coordinator (TOC) was notified.

An Air Canada Rouge Airbus A319-114 (C-GBHZ/ROU1651) from Miami Intl, FL (KMIA) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) declared PAN PAN. There was significant vibration in the control column. The aircraft landed without any problems. No impact.
UPDATE TSB report #A20Q0036: C-GBHZ, an Air Canada Rouge LP Airbus A319-114 (ROU1651), was on an instrument flight rules (IFR) flight from Miami, FL (KMIA) to Montreal Pierre Elliott Trudeau, QC (CYUL). While the aircraft was on final approach at 3 000 ft, the crew felt vibrations and heard loud bangs coming from the engine. The crew declared an emergency and landed without incident. No apparent damage was reported by the Aircraft Rescue and Fire Fighting (ARFF) crew during the ground inspection conducted once the aircraft had come to a stop on the runway. The aircraft then moved, without any other problems , to the gate for deplaning passengers.

Incident at 20NM NNW MONTRÉAL INTL (MIRABEL) QC (CYMX)

A Perimeter Aviation Fairchild SA227-AC (C-FYAG/PAG907) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Montreal/Pierre Elliott Trudeau, QC (CYUL) asked to be redirected to Montreal/Mirabel, QC (CYMX) due to an engine that would not start up again after tests were conducted. At that time, no emergency was declared. On final 5 NM Runway 24, the aircraft declared an emergency with the tower at CYMX. Aircraft Rescue and Fire Fighting (AARF) services were deployed. The aircraft landed at 2249Z without incident and a runway inspection was completed at 2255Z. No impact.
UPDATE TSB Report #A20Q0038: C-FYAG, a Fairchild SA227AC operated by Perimeter Aviation LP under flight number PAG907, was flying under instrument flight rules from Montreal/Pierre Elliott Trudeau International, QC (CYUL) airport on a local flight in order to perform flight tests. After voluntarily shutting down an engine, the crew was unable to restart it. The crew therefore requested to be redirected to Montreal/Mirabel, QC (CYMX), where they declared an emergency. The aircraft landed without incident.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Metro 3 aircraft C-FYAG, while on a flight test program, was carrying out single engine performance checks when the shutdown engine failed to restart. Flight crew declared an emergency and returned to the airport uneventfully. Maintenance performed a functional test of the right-hand (RH) feather valve, performed a functional test of the RH auto ignition system as per Maintenance Manual (MM) 72-10-06, performed a functional test of the RH engine unfeather system, and performed a functional test of the RH I/B squat switch. The RH engine unfeather pump output pressure was checked and found to be set at 180psi. The RH unfeather pump pressure was adjusted to 210psi.

An Air Algérie Airbus A330-202 (7TVJC/DAH2700) from Algiers, Algeria (DAAG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was instructed to conduct a go-around on Runway 06R because a Sky Regional Airlines Embraer ERJ 170-200 SU (C-FEIQ/SKV7742) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Newark, NJ (KEWR) had lined up on the runway while a Chartright Air Bombardier BD-100-1A10 (C-FXGO/HRT242) from Toronto/Lester B. Pearson, ON (CYYZ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was unable to exit via Taxiway L, which was blocked by a Government of Quebec Canadair CL600-2B16 (C-GQBQ/QUE20) on a MEDEVAC flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Quebec/Jean Lesage, QC (CYQB). No other impact on operations.

The pilot of a Jazz de Havilland DHC-8-402 (C-GKUK/JZA8753) from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Val-dOr, QC (CYVO) mentioned that it had a warning light linked to the windshield de-icing system système and requested a return to CYUL.

On final for Runway 06L, a Jazz de Havilland DHC-8-402 (JZA7960) from Toronto/Billy Bishop, ON (CYTZ) to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) reported the presence of smoke in the cockpit and declared an emergency. Coordination carried out. JZA7960 made it to the ground without incident.
UPDATE TSB Report #A20Q0035: C-GGOF, a Jazz Aviation LP Dash-8 402 (JZA7960), was on an IFR flight from Toronto/Billy Bishop Toronto City Airport, ON (CYTZ) to Montreal Pierre Elliott Trudeau, QC (CYUL). During the approach, the crew noticed light smoke near the right side of the windshield, as well as the smell of burnt electrical wires. The windshield heat system was turned off, which caused the smoke to dissipate. The crew declared an emergency and the aircraft landed without incident.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

At takeoff, an Air Canada Rouge Boeing 767-3Q8 (C-FJZK/ROU1726) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Pointe-à-Pitre, Guadeloupe (TFFR) reported that it had possibly hit something with an engine and requested a return to CYUL. ROU1726 declared an emergency. ROU1726 made it to the ground on Runway 06R at 1200Z without incident. Impact: Runway 06R/24L was unavailable from 1200Z to 1210Z. The National Operations Centre (NOC) and the Aviation Operations Centre (AVOPS) were notified.
UPDATE TSB Report #A20Q0034: C-FJZK, an Air Canada Rouge LP Boeing 767-3Q8 (ROU1726), had to conduct an instrument flight rules (IFR) flight from Montreal/Pierre Elliott Trudeau International, QC (CYUL) to Pointe-à-Pitre Le Raizet, Guadeloupe (TFFR). On initial climb, the crew heard a faint sound and observed an increase in engine vibrations. The air traffic controller told the crew that he had noticed what were probably flames coming out of the left engine during takeoff. The crew declared an emergency, turned around, and landed without incident. The Aircraft Rescue and Fire Fighting (ARFF) services crew was deployed and inspected the aircraft on the runway. There were no injuries.
