UPDATE TSB Report #A20O0026: Jazz Aviation LP, flight JZA949, a De Havilland DHC-8-400 (registration C-GGNW) aircraft, landed at Toronto / LBPIA (CYYZ) on Runway 24L and was instructed by ATC to take a right turn onto D2 and then cross Runway 24R. When the aircraft was approaching the holding position for Runway 24R, the flight crew observed an Air Canada ERJ 190-100 (flight AC462, registration C-FMZB) departing runway 24R in front of them. AC462 had received clearance from ATC to depart Runway 24R, and was on the take-off roll. The flight crew of JZA949 queried ATC when they saw AC462 passing in front. NAV Canada is conducting an internal investigation.

Accident at 14.5NM W SAINT JOHN NB (CYSJ)

TSB Report #A20A0019: A privately registered, amateur built Murphy Moose SR3500 (amphibian floats) was on a pleasure flight from Florenceville Airport, NB (CCR3) to Saint John Airport (CYSJ), NB. The pilot departed CCR3 at approximately 2015 ADT with approximately 15 gallons of fuel per tank. The aircraft was in cruise at 3000 feet ASL over the Saint John River, abeam Morrisdale, NB, when the engine began to run rough. The pilot checked the RH and LH fuel sight gauges and both tanks were indicating empty. All engine parameters were normal. The pilot planned a precautionary landing on the river between a powerboat operating in the area and a spit of land jutting out from the shoreline. At an altitude of 1500 feet AGL, the engine roughness increased and a complete power loss occurred a short time afterwards. The descent rate increased to approximately 900 FPM and the angle of glide steepened, requiring the pilot to bank to the right to remain clear of the powerboat. The aircraft stalled and departed controlled flight at approximately 100 feet AGL. It struck the surface of the water in a steep nose down right wing low attitude at approximately 2115 ADT. The aircraft cabin quickly filled with water and the pilot released himself from the three-point safety harness and exited the aircraft with an available life vest. The powerboat operator was on scene within minutes and recovered the pilot from the water. The pilot received minor injuries. Impact forces destroyed the aircraft. The aircraft was equipped with four life vests and four Helicopter Emergency Egress Device (HEED 3) units. The pilot had exited the aircraft before use of a HEED 3 was required. The pilot suspects fuel exhaustion as the initiating factor for the engine failure. A private aircraft maintenance company recovered the aircraft from the Saint John River. They will inspect the aircraft for evidence of a fuel related problem.

Accident at 15NM NW SAINT JOHN NB (CYSJ)

The National Operations Centre (NOC) called the Moncton area control centre (ACC) to advise and request information about a privately registered, amateur-built Murphy SR 3500. The aircraft had a crash landing in the St. John River at approximately 0015Z on June 14th. There was no known contact with this aircraft at the time of the accident. The pilot was injured in the accident.

Incident at SAINT JOHN NB (CYSJ)

A Sherob Management Ltd. Piper PA-31 (C-FRKG) from Fredericton Intl, NB (CYFC) to Saint John, NB (CYSJ) reported not having three green lights on final for Runway 32. The aircraft circled around to try to resolve the issue and then made a low pass on Runway 14. The flight service station (FSS) and two fire fighters in Aircraft Rescue and Fire Fighting (ARFF) trucks were able to observed 3 landing gears. The aircraft landed safely without any issues at 1739Z.
UPDATE Aviation Incident Report #16636: At 1720Z, the flight service station (FSS) called reporting an incoming Sherob Management Ltd. Piper PA-31 (C-FRKG) with a landing gear issue. A vehicle (Red3) took a standby position on the Vassey road and another vehicle (Red4) took up standby on the back road. The aircraft did a low fly-by to verify that the landing gear was in position for landing, as the pilot was getting no verification that the gear was locking into place. The aircraft landed on Runway 32 without incident, then was escorted to the Atlantic Flight Center by Red3 and Red4. The Aircraft Rescue and Fire Fighting (ARFF) unit returned to the hall.

A Government of Canada, Department of National Defence Lockheed P-3 (Aurora/Orion) (PATH01) from Greenwood, NS (CYZX) to Saint John, NB (CYSJ) was conducting training at CYSJ on Runway 23. At 1905Z, PATH01 was on the threshold of Runway 23, prior to departure, and advised shutting down Engine 1. A short time later, PATH01 advised taxiing to the apron to investigate the problem further. No impact to operations.

Incident at J.A. DOUGLAS MCCURDY SYDNEY AIRPORT

A Provincial Aerospace Beech 200 (C-GEHS/SPR810) from Halifax/Stanfield, NS (CYHZ) to Sydney, NS (CYQY) conducted a missed approach to Runway 36 due to an unsafe nose gear indication. The pilot then contacted the Moncton Centre and subsequently picked up an IFR clearance back to CYHZ. Aircraft returned to CYHZ and declared an emergency at 1940Z. Aircraft landed safely at 2015Z. A Craftsman Construction Ltd. Beech 95-B55 (C-FDMD) from Saint John, NB (CYSJ) to Halifax/Stanfield, NS (CYHZ) was delayed by 10-15 minutes. The Joint Rescue Coordination Centre (JRCC) Halifax was advised. (Duplicate AOR 263988-V1 deleted after merging information)
Update TSB Report #A20A0017: C-GEHS, a Beech 200 aircraft operated by Provincial Aerospace, was conducting flight SPR810 from Halifax/Stanfield Intl, NS (CYHZ) to Sydney/J.A. Douglas McCurdy, NS (CYQY). During the approach to CYQY, the crew lowered the landing gear and noticed an unsafe nose gear condition. At approximately 15:07 AST, the crew conducted an overshoot and elected to return to CYHZ. The crew declared an emergency at 15:40 AST and ACC advised the Rescue Coordination Center and the CYHZ Airport Rescue and Fire Fighting (ARFF). The aircraft landed uneventfully at 16:15 AST with ARFF standing by. One aircraft was delayed for approximately 15 minutes before it could land. Maintenance examined the aircraft NLG system and determined that the nose gear down lock switch had failed. They installed a new switch, completed several successful landing gear swings and returned the aircraft to service.

C-FMGC departed at 1050Z and confirmed that there were two geese at the location observed by the FSS.

Gear lowered manually. The aircraft landed safely. No impact on operations.
UPDATE TSB Report #A20A0012: C-FMGC, a privately operated Beechcraft B200GT aircraft, was conducting a flight under instrument flight rules from Saint John Airport (CYSJ), NB to Halifax/Stanfield Intl (CYHZ), NS. On approach to CYHZ, the crew selected the landing gear down and the landing gear indication did not change, nor did it move from the up position. The crew conducted a missed approach and communicated the condition to CYHZ tower. The crew followed the QRH for landing gear failure and conducted a manual landing gear extension, which successfully lowered the gear. The crew then received landing clearance from CYHZ tower and requested that emergency services standby. The aircraft landed at CYHZ without further incident. Maintenance personnel replaced a to rectify the fault.

A Jazz Bombardier CL-600-2B19 (C-FIJA/JZA8788) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Saint John, NB (CYSJ) was about to turn a 10 mile final for Runway 05 in CYSJ when they received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) for an unknown aircraft 500 feet below them. The aircraft only became visible to flight service station (FSS) on radar after JZA8788 reported the RA. At the time JZA8788 was at 2400ft and the unknown aircraft was at 1900ft. The aircraft was squawking 1200 and appeared to be flying southwest bound along the coast. JZA8788 was able to continue the approach and landed safely at 1341Z.

A Jazz de Havilland DHC-8-301 (C-GMON/JZA8791) from Saint John, NB (CYSJ) to Montreal/Trudeau, QC (CYUL) decided to conduct a go-around. Several arrivals were redirected to Runway 24L. Several departures from Runway 24L were therefore delayed. A duck was found during the inspection of the runway. JZA8791 landed on Runway 24L at 1107Z.

A de Havilland DHC-8-402 operated by Jazz (C-GUJZ/JZA8792) from Montreal/Trudeau International (CYUL), QC, to Saint John (CYSJ), NB, declared an emergency because of a fire in the cockpit. The aircraft landed safely at 0129Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During climb, the Cabin Crew observed abnormal smoke originating from the outboard coffee maker along with a strong smell. After notifying the Flight Crew, the associated circuit breakers were pulled and an orange color glow was observed behind the coffee maker. A portable fire extinguisher was discharged along with a 1.5 liter water bottle. The Flight Crew declared an emergency and executed an air turn back without further incident. The coffee maker and the portable fire extinguisher were replaced before returning the aircraft to service. A Service Difficulty Report (SDR) was submitted. Note: Sugar packets were found behind the coffee maker; however, there was no evidence that it contributed to the incident.
Update - TSB Report #A19Q0003: C-GUJZ, a Jazz Aviation LP Bombardier DHC-8-400, was on instrument flight rules (IFR) flight JZA8792 from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Saint John, NB (CYSJ) with 4 crew members and 66 passengers on board. Approximately 45 NM east of CYUL, when the aircraft was climbing past 16 000 ft ASL, the cabin crew noticed smoke as well as a reddish glow behind the coffee maker. With the use of an extinguisher, the fire was brought under control. The flight crew members put on their oxygen masks, declared an emergency, and requested a return to CYUL, where the aircraft landed without any problems. The aircraft taxied up to the walkway, from which passengers were able to exit the plane. No one was injured and the aircraft sustained no damage.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown, FOD (foreign object debris))

An Ocean Aviation Services Bombardier BD-100-1A10 (C-GJEI) from Saint John, NB (CYSJ) to Toronto/Lester B. Pearson, ON (CYYZ) reported bird strike upon landing on Runway 24L. Vehicle (BMN74) responded and reported one intact bird on the runway. Runway 24L closed from 1245Z till 1252Z for BMN74 inspection. As a result, a GoJet Airlines Bombardier CL-600-2C10 (N642CA/GJS6101) from Detroit Metro/Wayne County, IL (KDTW) to Toronto/Lester B. Pearson, ON (CYYZ) and a WestJet Boeing 737-7CT (C-FMWJ/WJA1203) from New York-LaGuardia, NY (KLGA) to Toronto/Lester B. Pearson, ON (CYYZ) were instructed by air traffic services (ATS) to go around. Both landed at 1259Z and 1302Z respectfully, without further issue (WOFI). No Operational Impact.

A Jazz de Havilland DHC-8-402 (C-GGNZ/JZA8972) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Saint John, NB (CYSJ) departed CYYZ and when climbing through 3500 ft, reported being illuminated by green laser approximately 6NM south of CYYZ.
