A Jazz de Havilland DHC-8-402 (C-GGOY/JZA8862) from Halifax/Stanfield, NS (CYHZ) to Gander, NL (CYQX) returned to CYHZ with a mechanical issue. No assistance was required. The Aircraft Rescue and Fire Fighting (ARFF) unit was advised and on standby for the aircrafts landing. JZA8862 landed safely at 1523Z with no impact on operations.

Incident at GANDER INTL NL (CYQX)

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: The pilot of a Jazz de Havilland DHC-8-402 (C-GGBF/JZA8863) from Gander, NL (CYQX) to Halifax/Stanfield, NS (CYHZ) departed from Runway 03 at 2120Z and, shortly after take-off, contacted the tower to ask if any rubber had been left on the runway after their departure, because they had experienced some shaking with the nose landing gear during take-off. The field foreman for the airport inspected the runway and found no evidence of any debris. The tower contacted the aircraft and relayed the ground crews report. No impact to operations.

The Area Control Centre (ACC) advised the Tower that a Search and Rescue (SAR) helicopter, a Government of Canada, Department of National Defence Cormorant CH-149 (OCST909) from Gander, NL (CYQX) to Gander, NL (CYQX), was experiencing an issue with its autopilot and was returning to CYQX. The aircraft landed without incident, and with no assistance required.

Due to a medical emergency, a Cargolux Airlines International Boeing 747-800 (LXVCL/CLX7416) from Guadalajara, Mexico (MMGL) to Luxembourg, Luxembourg (ELLX) requested to divert to Gander, NL (CYQX). At 1700Z, the flight was cleared as requested. No impact to operations. Flight landed at 1755Z. (Duplicate CADORS 2020A0344 deleted).
Update TSB Report #A20A0015: LX-VCL, a Boeing 747-800 aircraft operated by Cargolux International, was conducting flight CLX7416 from Guadalajara/Don Miguel Hidalgo Y Costilla Intl. (MMGL), MX, to Luxembourg-Findel Intl.(ELLX), LU. Upon entering the Gander Flight Information Region, the crew made a PAN PAN call due to a medical emergency. A relief crew member was becoming progressively ill. The crew requested and were cleared direct to Gander Intl (CYQX), NL. The aircraft landed uneventfully on Runway 21 at 1755 UTC. Emergency Health Services met the aircraft on the ramp and transported the relief crew member to the hospital. The aircraft departed CYQX the following day.

Flight was cleared as requested. At 0047Z, 90NM northeast of CYQX, RCH144 declared an emergency. No impact to operations and landed without incident at 0106Z.

The controller advised the aircraft that they were required to level off for a couple of minutes for traffic, a Jazz de Havilland DHC-8-402 (C-GGOI/JZA8878) from Halifax/Stanfield, NS (CYHZ) to Gander, NL (CYQX). JZA8878 readback the second call and levelled at 10 000ft. No impact on operations.

Incident at 195NM NE ST. JOHN'S INTL NL (CYYT)

At time 2202Z and just west of 50N050W, a British Airways Boeing 787-9 (GZBKD/BAW293) on a flight from Heathrow/London Intl, UK (EGLL) to Washington/Dulles, VA (KIAD) declared a PAN PAN due to a pressurization issue and requested descent from FL410 to FL350. The flight was recleared to FL340. BAW293 reported the situation good at FL340 and PAN PAN cancelled at 2215Z. Flight continued to its destination and exited the Gander Intl, NL (CYQX) flight information region (FIR) at 2248Z.
UPDATE TSB Report #A20A0006: G-ZBKD, an Boeing 787-900 operated by British Airways, was conducting flight BAW293 from London Heathrow Intl. (EGLL), UK to Washington Dulles Intl. (KIAD), VA. The aircraft dispatched with the right air conditioning pack inoperative in accordance with the MEL. During the cruise portion while at FL410, the left air conditioning pack failed, resulting in a PACK ALTITUDE LIMIT warning. The checklist required a descent to FL340. At 195 nm NE of St John’s International Airport, NL (CYYT), the crew requested FL340, however it was denied by ATC. The crew then declared a PAN-PAN due to the pressurization issue and a descent to FL340 was made. Once the aircraft reached FL340, the crew reported the situation was normal and cancelled the PAN-PAN and continued to destination without incident.

Kennedy, NY (KJFK) was diverting to Gander Intl, NL (CYQX) due to a medical emergency on board. The aircraft landed safely at 1050Z. Ambulance at airport on standby when aircraft landed.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

After landing Runway 14 and vacating in Taxiway M, an Exploits Valley Air Services Beech 1900D (C-GLHO/EV121) on a flight from Gander Intl, NL (CYQX) to Halifax/Stanfield Intl, NS (CYHZ) was told to taxi via D and hold short of Runway 23. A Jazz de Havilland DHC-8-402 (C-GGNW/JZA8635) on a flight from Halifax/Stanfield Intl, NS (CYHZ) to Ottawa/MacDonald-Cartier Intl, ON (CYOW) was rolling Runway 23 as EV121 was observed crossing the hold short line and entering the runway protected area. Tower controller instructed JZA8635 to abort takeoff and JZA8635 was able to stop prior to Taxiway D.
UPDATE TSB Report #A20A0005: C-GLHO a Beechcraft 1900D aircraft operated by Exploits Valley Air Services (EVAS) was conducting flight EV121 from Gander Intl (CYQX), NL to Halifax Stanfield Intl (CYHZ), NS, which was a maintenance repositioning flight with crew only. After landing on runway 14 the EVAS crew was cleared to taxi "Delta" to hold short of runway 23 and read it back. C-GGNW, a de Havilland Dash 8-400 aircraft operated by Jazz, was conducting flight JZA8635 from CYHZ to Ottawa Intl (CYOW), ON, with 78 passengers. As JZA8635 accelerated on runway 23 for departure the crew noted that EV121 did not stop as directed and continued as if to cross in front of the departing aircraft. JZA8635 applied braking just as the tower controller told them to reject the takeoff. The ground controller also called EV121 directing them to stop. JZA8635 stopped about 30 meters from the intersection, EV121 stopped just short of entering the active runway. The Jazz flight taxied back to runway 23 and departed for CYOW. EVAS has conducted an internal SMS review.

At 1736Z a United Kingdom Royal Air Force Transport 9402 Airbus A330-200 (ZZ337/RRR9402) from Gander, NL (CYQX) to Halifax/Stanfield, NS (CYHZ) requested course change to make "gross weight adjustment". Once clear of other traffic, aircraft started dumping fuel at 1742Z on south-west track ending at 1814Z. No emergency, and no impact on other traffic.

At LEVAD the controller advised that the tower observed visibility was less than 1/2SM. When the aircraft was on short final, it was noticed that approach lights for Runway 31 were not on, lights turned on immediately.

Update TSB #A19A0034: C-FMXC, a Boeing 767-300 aircraft operated by Air Canada rouge, was conducting flight ROU1913 from Barcelona/EI Prat (LEBL), Spain to Montreal/Pierre Elliot Trudeau Intl (CYUL), QC with 8 crew members and 230 passengers on board. During the cruise portion of the flight in the vicinity of Gander Intl (CYQX), NL, a flight attendant became incapacitated due to illness and was unable to perform duties. Positions were reassigned for the landing, and the flight landed at destination without further incident.

The low domestic controller advised the Gander, NL (CYQX) tower controller that the aircraft was experiencing "navigational issues" and was returning to the airport, but required no assistance. The aircraft landed safely at 2210Z. No impact on operations.

A VFR aircraft reported that, after departure, the door had opened and that they were unable to close it. The pilot requested to join the circuit at Gander, NL (CYQX) for a "stop and go" so that they could resolve the issue before continuing with their flight plan. The aircraft landed and the pilot was able to close the door. The aircraft departed without incident. No impact on operations.

TSB Report #A19A0092: C-GORI a Beechcraft 1900D aircraft operated by Exploits Valley Air Services was conducting flight EV7784 from Gander Intl (CYQX), NL to St Johns Intl. (CYYT), NL with 2 crew and 11 passengers. On approach to CYYT, the aircraft was intercepting the final approach course at 3000 feet when severe turbulence was encountered. The crew had difficulty controlling the aircraft, however the turbulence abated as they continued their descent. A passenger fainted, and was treated by EHS upon landing. The turbulent air inspection was carried out by maintenance and no faults were found, the aircraft was released to service.

Controller could read pilot loud and clear but the pilot was not receiving their transmissions. The aircraft was then vectored back for the approach and landed safely with no issues on the second approach.
