Incident at 85NM S Winterton, NL

At 1220Z, two VFR aircraft within 25NM of Gander, NL (CYQX) reported hearing a "MAYDAY" call on the enroute frequency from a privately registered, amateur-built Murphy Rebel float plane near Winterton, NL. They informed the tower controller that the float plane was in contact with a rescue helicopter. No impact on airport operations.

A Novajet Dassault Falcon 900EX (C-FZJS/NOJ91) from Halifax/Stanfield, NS (CYHZ) to Gander, NL (CYQX) requested to stop its climb at FL300 due to an oil pressure issue. The aircraft declared "PAN PAN" with a #1 engine shutdown and continued to its destination. No assistance required, no operational impact. The flight landed safely at 1625Z.

An Exploits Valley Air Services Cessna 152 training aircraft (C-FPYE) from Gander Intl, NL (CYQX) to Gander Intl, NL (CYQX) was given takeoff clearance off Runway 21 from Taxiway Foxtrot. Moments later, the tower observed the aircraft airborne off Runway 03. Two aircraft, including an Exploits Valley Air Services Piper PA-34-200 (C-GHSE), were given alternate instructions to avoid conflict. C-FPYE was sequenced to land full stop and taxied back to the flight school without further incident.

Incident at 30NM NE GANDER INTL NL (CYQX)

Approximately 30NM northeast (NE) of Gander, NL (CYQX), a Provincial Aerospace Beech 200 (C-GPGR/SPR06) from St. Johns, NL (CYYT) to St. Anthony, NL (CYAY) requested to return to CYYT, with no reason provided. The aircraft subsequently declared an emergency due to an oil leak in the #1 engine. The aircraft landed without incident. No impact to operations.
UPDATE: TSB Report #A21A0026: C-GPGR, a Beech King Air BE20 aircraft operated by Provincial Aerospace Ltd., was conducting surveillance flight SPR006 from St. John’s, NL (CYYT) to St. Anthony, NL (CYAY). An hour and a half into the flight, the crew decided to return to CYYT, due to an onboard radar equipment issue. While planning the approach, the crew noticed fluid streaming from the No.1 engine, behind the propeller and the vent from the top of the cowling. All engine indications were normal. Due to another inbound aircraft that had requested priority due to a medical emergency, the crew decided to declare an emergency to avoid a delay. The aircraft landed without incident and taxied to the ramp. Maintenance inspected the No.1 engine and discovered oil leaking from the engine torque transmitter and a damaged O-ring. The O-ring was replaced, a ground run was completed and the aircraft was returned to service.

Incident at GANDER INTL NL (CYQX)

An Exploits Valley Air Services Ltd. Cessna 172N (C-GVKT) from Gander, NL (CYQX) to Gander, NL (CYQX) was conducting circuits on Runway 21. The solo pilot was instructed to conduct a low approach, restricted to "not below 1000 ASL" due to a vehicle on the opposite end of the runway. The aircraft was observed breaking the altitude restriction and by the time the aircraft was instructed to pull up, the aircraft was nearing touch down and continued to land. C-GVKT was nowhere in close proximity to the ground vehicle when it landed on the runway, and the student pilot was instructed to taxi back to the hangar.

UPDATE: TSB Report #A21A0014: C-FNOE, a Boeing 787-9 aircraft operated by Air Canada, was conducting flight AC898 from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Athens, Greece (LGAV). While in cruise flight about 370 NM east of Gander, NL (CYQX), a flight attendant became ill and was unable to perform the duties of the position. The crew declared an emergency with Gander Centre and requested clearance to divert to CYQX. An amended route clearance was issued and the flight landed at CYQX without incident. The flight attendant was transported to the hospital, and the flight departed for its original destination employing minimum cabin crew compliance procedures.

PHA162 requested to divert to Gander, NL (CYQX) and did not declare an emergency, nor requested any assistance. There was no impact to operations and the flight landed at 1514Z.

Moments after the takeoff clearance was given, the aircraft advised that it needed to taxi back to the main apron due to another indication. No impact on operations.

The aircraft advised that the issue was resolved and requested a diversion to St. Johns, NL (CYYT). Subsequently, the aircraft declared an emergency and landed without incident in CYYT. No impact on operations.
UPDATE: TSB Report #A21A0003: C-GNLO, a Government of Newfoundland and Labrador Beech B300C, was conducting a flight from Deer Lake, NL (CYDF) to Gander, NL (CYQX). On approach to CYQX, the landing gear was selected "down" but the crew did not receive a "left-hand main landing gear down" green indication. The crew cancelled their IFR clearance, carried out the checklist items, and consulted with their maintenance department. It was decided to divert to St. Johns, NL (CYYT), where an emergency was declared in order to have emergency services standing by as a precaution. The aircraft landed at CYYT without further incident. A faulty "left-hand main landing gear down" indication lamp was replaced, and the aircraft was returned to service following operational checks.
