Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

TSB Report #A20Q0024: C-GABO, a Jazz Aviation LP Dash-8-300 (DH8C) (JZA8970), was on an IFR flight from Ottawa/Macdonald-Cartier, ON (CYOW) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 48 passengers and 3 crew members on board. After the landing and while the aircraft was taxiing to the terminal, a passenger alerted the crew to flames coming out from the right main gear. The crew brought the aircraft to a stop and requested the intervention of Aircraft Rescue and Fire Fighting (ARFF) services. The fire quickly went out and the aircraft was towed away to the passenger bridge. When the aircrafts right main gear was checked, a breakage was found in two bearings in the inboard wheel.

Incident at 5233N / 07406W

An automatic dependent surveillance – contract (ADS-C) report indicated that a CargoJet Airways Ltd. Boeing 767-323 (C-GAJG/CJT470) from Ottawa/Macdonald-Cartier, ON (CYOW) to Iqaluit, NU (CYFB) was not following the route in its flight plan, which was YFM 60N71W YFB. The report instead indicated that the aircraft was following YFM YFB. No impact on operations.

Upon landing on Runway 24L, a Jazz de Havilland DHC-8-402 (C-GGNW/JZA8949) from Saint John, NB (CYSJ) to Toronto/Lester B. Pearson, ON (CYYZ) was cleared to taxi via Taxiway D2 and cross Runway 24R. Separation was not assured with a departing Air Canada Embraer ERJ 190-100 IGW (C-FMZB/ACA462) from Toronto/Lester B. Pearson, ON (CYYZ) to Ottawa/Macdonald-Cartier, ON (CYOW). Transportation Safety Board of Canada (TSB) advised.

UPDATE TSB Report #A20O0026: Jazz Aviation LP, flight JZA949, a De Havilland DHC-8-400 (registration C-GGNW) aircraft, landed at Toronto / LBPIA (CYYZ) on Runway 24L and was instructed by ATC to take a right turn onto D2 and then cross Runway 24R. When the aircraft was approaching the holding position for Runway 24R, the flight crew observed an Air Canada ERJ 190-100 (flight AC462, registration C-FMZB) departing runway 24R in front of them. AC462 had received clearance from ATC to depart Runway 24R, and was on the take-off roll. The flight crew of JZA949 queried ATC when they saw AC462 passing in front. NAV Canada is conducting an internal investigation.

A WestJet Boeing 737-800 (WJA4306) from Ottawa/Macdonald-Cartier, ON (CYOW) to Toronto/Lester B. Pearson, ON (CYYZ) was initiated by the tower to conduct an overshoot (O/S) of Runway 23. A SkyWest Airlines Bombardier CL-600-2B19 (SKW4463) from Toronto/Lester B. Pearson, ON (CYYZ) to Minneapolis/St-Paul, MN (KMSP) was on the runway and was given take-off clearance, but the aircraft was not rolling. SKW4463 then asked the tower if they were cleared, at which point WJA4306 was given the overshoot. WJA4306 landed at 0031Z. No operational impact.

Incident at RONTA intersection

A Jazz Bombardier CL-600-2D24 (JZA7917) from Raleigh-Durham, NC (KRDU) to Toronto/Lester B. Pearson, ON (CYYZ) was instructed to fly a heading of 030 to intercept the Instrument Landing System (ILS) for Runway 05, and read back the same. When approaching the localizer (LOC), JZA7917 was observed beginning a left hand turn and the crew advised they had bad tracking. The controller cancelled the approach clearance for an Air Canada Rouge Airbus A319 (ROU1981) from Mexico City, Mexico (MMMX) to Toronto/Lester B. Pearson, ON (CYYZ), which was the next aircraft in sequence, and instructed an Air Canada Embraer ERJ 190-100 IGW (ACA475) from Ottawa/Macdonald-Cartier, ON (CYOW) to Toronto/Lester B. Pearson, ON (CYYZ) to climb back to 4000 feet. The controller issued vectors to JZA7917 in order to re-sequence them. There were no conflicts and JZA7917 landed uneventfully at 2334Z.

Incident at OTTAWA / CARP ON (CYRP)

Aviation Incident Report #16542: At approximately 1515Z, a PAN PAN call was received from an Ottawa Flying Club Cessna 150M (C-GFXL) on a flight from Ottawa/Macdonald-Cartier Intl, ON (CYOW) to Ottawa/Macdonald-Cartier Intl, ON (CYOW) stating that they had no oil pressure indicating on their gauge. The aircraft was able to land safely at Ottawa/Carp, ON (CYRP), taxi clear of the runway, and shutdown on the apron by a snow bank. Examination of the engine compartment revealed that something on the Left Hand side had let go, and oil had sprayed all across the left side of the engine, lower left engine cowling, and parts of the left hand under-fuselage. Aircraft remains at CYRP pending maintenance by Ottawa Flying Club.

A Jazz Bombardier CL-600-2D24 (C-GPJZ/JZA8634) on a flight from Ottawa/MacDonald-Cartier Intl ON (CYOW) to Halifax/Stanfield Intl, NS (CYHZ) and a Jazz de Havilland DHC-8-311 (C-GABO/JZA8659) on a flight from Ottawa/MacDonald-Cartier Intl, ON (CYOW) to London, ON (CYXU) were delayed several minutes.

An Air Canada Airbus A220-300 (ACA2334) on a flight from Winnipeg/James Armstrong Richardson Intl, MB (CYWG) to Ottawa/MacDonald-Cartier Intl, ON (CYOW) reported a large drone in close proximity to the aircraft on 3NM final Runway 25 at 1500ft. Airline Operations Control Centre (AOCC) advised. No impact on operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A WestJet Boeing 737-700 (WJA4306) from Ottawa/Macdonald-Cartier, ON (CYOW) to Toronto/Lester B. Pearson, ON (CYYZ) was instructed by the tower to conduct an overshoot of Runway 23. A SkyWest Airlines Bombardier CL-600-2D24 (SKW4463) from Toronto/Lester B. Pearson, ON (CYYZ) to Minneapolis-St. Paul, MN (KMSP), on the runway, had been given take-off clearance but was not rolling. The pilot of SKW4463 then asked tower if they were cleared, at which point WJA4306 was given the overshoot instructions. WJA4306 landed at 0031Z. No operational impact.
