The Fort McMurray, AB (CYMM) tower was providing Class C air traffic service (ATS) service on March 19 from 0445Z until 1215Z due to local communication equipment failure.

March 15, 2020

Incident at 5528N / 11129W

A WestJet Encore de Havilland DHC-8-402 (WEN3149) from Calgary, AB (CYYC) to Fort McMurray, AB (CYMM) declared PAN PAN due to a medical emergency on board, and requested medical assistance at the gate. The aircraft landed at 0656Z. No impact to operations.

Incident at 20NM E EDMONTON INTL AB (CYEG)

A Sunwest Aviation Learjet 35A (C-GTJL/CNK633) MEDEVAC from Winnipeg/James Armstrong Richardson, MB (CYWG) to Edmonton, AB (CYEG) was 20NM East of CYEG and declared Mayday due to an engine failure. Emergency services were arranged. CNK633 landed safely on Runway 30 at 0146Z. A WestJet Encore de Havilland DHC-8-402 (WEN3132) from Fort McMurray, AB (CYMM) to Edmonton, AB (CYEG) was delayed to allow CNK633 and emergency vehicles to clear the runway.
UPDATE TSB #A20W0023: The Sunwest Aviation Learjet 35, C-GTJL, operating as CNK633 on a flight from Winnipeg (CYWG), MB to Edmonton (CYEG), AB was on a descent through 12 000 feet when the left hand engine (TFE-731-2-2B) started to lose power. The throttle was advanced to try and regain power but the engine power continued to decrease. As the engine continued to lose power, the associated low oil press light came on and triggered the master warning. The crew declared a Mayday and asked for delaying vectors so they could run the associated checklists. Once the engine was secured, the flight crew told ATC they could continue inbound for a single-engine approach and landing. ARFF was requested to stand by for the landing. The landing and taxi to the FBO happened without further incident. Update: The engine was inspected and motored with no faults found. The digital electronic engine control (DEEC) had recorded the engine shutdown but there were no other indications of a fault. The aircraft had been over-wing refueled in Winnipeg (CYWG) MB by two trucks; one on each wing. The fuel request showed that additive was requested and both trucks were capable of providing the same. The aircraft was refueled after the maintenance checks in CYEG and engine test runs were undertaken with no faults found. The aircraft was then flown under a ferry permit back to Calgary (CYYC), AB for further investigation.

An Alberta Central Airways Ltd. Beech B200 (C-FSUG) from Fort McMurray, AB (CYMM) to Edmonton, AB (CYEG), on the approach for Runway 30, reported a green laser strike. Royal Canadian Mounted Police (RCMP) were advised. No impact to the operation.

Incident at FORT MCMURRAY AB (CYMM)

McMurray aviation dispatch called into tower to report a McMurray Aviation Cessna U206G (C-GMFV) from Fort Chipewyan, AB (CYPY) to Fort McMurray, AB (CYMM) inbound from the North with no transponder or radio. Dispatch told pilot to phone tower directly, but pilot was unable to phone the tower land line. Instructions were given to dispatch to relay to pilot about what runway to use, altitude to stay at, and part of circuit to enter. Tower made visual contact with aircraft, light gunned the pilot to land, as well as relayed instruction through dispatch with taxi instructions back to hanger. Aircraft landed at 1450Z, no impact to operations.

Incident at FORT MCMURRAY AB (CYMM)

At Fort McMurray, AB (CYMM), a McMurray Aviation Cessna 172S (C-FXVA) was instructed to follow a McMurray Aviation Cessna 172L (C-GYYE) in the circuit. Reported traffic in sight. C-FXVA was observed overtaking C-GYYE and was instructed to do a left 360 for spacing. C-GYYE reported risk of collision with C-FXVA while turning base. No operational impact.

Incident at FORT MCMURRAY AB (CYMM)

At Fort McMurray, AB (CYMM), an emergency locator transmitter (ELT) alarmed on 121.5 for 1 minute. Shortly after, a phone call was received from the pilot of a privately registered Glasair Sportsman 2+2 to notify that they accidentally triggered their ELT in the hangar. Air Traffic Operations Specialist (ATOS) advised.

A Central Mountain Air Ltd. de Havilland DHC-8-314 (GLR9405) from Edmonton, AB (CYEG) to Fort McMurray, AB (CYMM) reported a pressure issue. Not an emergency. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. GLR9405s sticky outflow valve was replaced in accordance with (IAW) Aircraft Maintenance Manual (AMM) 21-30-11. Pressurization control tested IAW AMM 21-30-00. Found serviceable. Minimum Equipment List (MEL) #21-1 cleared and placard removed.

Incident at FORT MCMURRAY AB (CYMM) (Aerodrome, runway or taxiway shutdown)

A Northern Air Charter Inc. Beech 200 (NLI777) from Fort Chipewyan, AB (CYPY) to Fort McMurray, AB (CYMM) landed Runway 08 at CYMM at 1546Z. NLI777 had a "frozen brake" and two of their tires blew, rendering them immobile on the runway. Maintenance vehicles were eventually able to move aircraft off the runway but a Jazz de Havilland DHC-8-402 (JZA8372) from Fort McMurray, AB (CYMM) to Calgary, AB (CYYC) was delayed approximately one hour. No other impact to the operation.

Incident at FORT MCMURRAY AB (CYMM)

A WestJet Boeing 737-8CT (C-GWSZ/WJA4012) enroute from Edmonton Intl, AB (CYEG) to Fort Mackay/Firebag, AB (CYFI) declared an emergency due to smoke in the cockpit at 0144Z. The aircraft diverted to Fort McMurray, AB (CYMM) and landed safely at 0155Z. No impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The aircraft was confirmed to be C-GWSZ. Flight crew reported smoke in the cockpit on log page AK61696 while en-route from CYEG to CYFI and diverted to CYMM. WestJet maintenance inspected the cockpit for evidence of smoke and found evidence of overheating on the First Officer’s Audio Control Panel (ACP) (PN 5145-1-77 Boeing Vendor V30242 AvTech Tyee Inc.) faceplate. The ACP from the Observers position was swapped into the First Officer’s position in accordance with (IAW) Boeing aircraft maintenance manual (AMM) 23-51-02 for testing and further troubleshooting, and tested serviceable with no faults found. An ACP was sent up from CYYC and the ACP was installed in the Observers position IAW Boeing AMM 23-51-02. The Observers position ACP could not be tested fully at the time so the observers position ACP was placed on Minimum Equipment List (MEL) 23-13-04. Due to the O2 masks being donned in flight, the crew O2 levels were just above dispatch minimums for two flight crew members, so the 1st and 2nd cockpit Observers seats were placed on MEL 25-11-01 and 02, respectively, on Log Page AK61698 and AK61699 to ensure they were not occupied. The aircraft was dispatched and flew to CYEG. In CYEG, the O2 bottle was serviced IAW Boeing AMM 12-15-21 and MELs 25-11-1 and -2 were removed. The Observers ACP was tested serviceable IAW Boeing AMM 23-51-02 and MEL 23-13-04 was removed. The ACP will be sent to Avtech Tyee FAA Repair Station IG6R621N for inspection and repair.
UPDATE TSB Report #A20W0001: C-GWSZ, a WestJet Boeing 737-800, was operating as flight WS4012 from Edmonton (CYEG), AB to Fort Mackay / Firebag (CYFI), AB. During cruise the flight crew noticed a burning smell in the flight deck. The crew declared an emergency and diverted to Fort McMurray (CYMM), AB. The aircraft landed without further incident. Company maintenance found that the source of smoke was from the first officers audio panel. The audio panel was replaced and the aircraft was returned to service.

Douglas DC-9-30 (JUS192) from Ypsilanti/Willow Run, MI (KYIP) to Hamilton, ON (CYHM) was delayed by approximately 10 minutes.

UPDATE TSB Report #A19W0167: The Air Canada Express De Havilland Dash 8-400, C-GJZD, was operating as JZA374 from Fort McMurray (CYMM), AB to Calgary (CYYC), AB. While en route, the flight crew observed a Deice Pressure caution indication. The crew flew an increased approach and landing speed and declared an emergency. ARFF was on standby for the arrival which was carried out without incident. Update: Wing boot and deice system checked in all modes by maintenance personnel. All systems checked normal and the aircraft was returned to service.

Update TSB Report #A19W0158: C-GGNF, Jazz Aviation LP de Havilland DHC-8-400 was operating as flight JZA8382 from Edmonton (CYEG), AB to Fort McMurray (CYMM), AB. While enroute to CYMM (100NM NNE CYEG), the flight crew observed a cabin pressure indication and initiated an emergency descent. The aircraft descended to 14 000 feet ASL and was further cleared to descend to 10 000 feet ASL. The flight crew elected to return to CYEG. No emergency was declared and the aircraft landed without further incident. There were no injuries. Company maintenance replaced the cabin pressure control.

Aviation Incident Report #16229: A passenger aboard a WestJet flight to Fort McMurray, AB (CYMM) was intoxicated and using a vaporizer during the flight. Passenger was not complying with flight crews instructions and was filming the flight attendants. Flight crew issued a notice from the captain. The Royal Canadian Mounted Police (RCMP) were contacted to meet the flight upon arrival.

Traffic information was passed on a WestJet Encore de Havilland DHC-8-402 (WEN3132) on a flight from Fort McMurray, AB (CYMM) to Edmonton Intl, AB (CYEG) at FL240. WJA4013 was cleared to FL250. WJA4013 reached FL260 and responded to a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and climbed 300 feet.

Aerospatiale AS 350 B-2 (C-FOQA) on a flight from Fort McMurray, AB (CYMM) to Fort McMurray, AB (CYMM) hovering west of the threshold of Runway 08 was told to stay west of the Taxiway B intersection for aircraft taxiing across Runway 08. C-FOQA was observed hovering eastbound through Taxiway B intersection. Impact: C-FOQA was told to continue eastbound to avoid conflict with taxiing aircraft, a Jazz de Havilland DHC-8-102 (C-GKON/JZA8385) on a flight from Fort McMurray, AB (CYMM) to Edmonton Intl, AB (CYEG).

A Northern Air Charter Beech 200 (NLI373) enroute from Calgary Intl, AB (CYYC) to Fort McMurray, AB (CYMM) declared an emergency with smoke in the cockpit. Ended up being a minor issue, and the aircraft arrived without incident at 1559Z. No impact on operations. (Duplicate AOR 257967-V1 deleted).
Update TSB Report #A19W0144: C-GSIA, a Northern Air Charter (P.R.) Inc. Beechcraft B200 was operating as flight NLI373 from Calgary International (CYYC), AB to Fort McMurray (CYMM), AB. Just prior to descent into CYYM the flight crew noticed smoke coming from the co-pilots subpanel. All non-essential electrics were turned off and the smoke cleared. An emergency was declared and ARFF was standing by for the landing in CYMM. The aircraft landed without further incident. Following the flight, maintenance determined that the smoke was caused by a defective blower motor. The motor was deferred and the aircraft was flown (crew only) to CYYC for further maintenance.

A SkyWest Airlines Canadair RJ 200 Regional Jet (N1385Y/SKW5956) from Edmonton, AB (CYEG) to Denver, CO (KDEN) was issued taxi instructions for Runway 12 via A and B. The tower controller gave SKW5956 takeoff clearance for Runway 12 as the aircraft was approaching the Alpha Bravo intersection. A Syncrude Canada Cessna 650 Encore (C-FSNC/SYN561) from Fort McMurray, AB (CYMM) to Edmonton, AB (CYEG) was on its 5NM final for Runway 20. Tower controller observed SKW5956 taxiing too fast to make the left turn onto Bravo. Tower advised SKW5956 to hold short, SKW5956 stopped, and tower identified that SKW5956 was indeed taxiing for Runway 02. SKW5956 subsequently had room to make the left turn onto Bravo and departed Runway 12. No impact to operations.

A WestJet Encore de Havilland DHC-8-402 (WEN3146) on a flight from Fort McMurray, AB (CYMM) to Calgary Intl, AB (CYYC) and a WestJet Encore de Havilland DHC-8-402 (WEN3346) on a flight from Edmonton Intl, AB (CYEG) to Calgary Intl, AB (CYYC) were operating in the same general area both inbound landing in Calgary. WEN3146 was advised to be aware that WEN3346 was also on the same frequency. A clearance which was directed to WEN3346 was readback by WEN3146. The controller immediately advised WEN3146 that the descent clearance was not for them and advised them to confirm they were maintaining FL240. WEN3146 confirmed holding at FL240, no impact to operations.

Incident at FORT MCMURRAY AB (CYMM)

At 2023Z, a W.M.K. Holdings Ltd. Cessna U206E (CGHJB) on a flight from Fort McMurray, AB (CYMM) to Fort Smith, NT (CYSM) requested to return to CYMM due to an alternator issue. The flight landed without incident at 2035Z. No impact to operations.
