Incident at 5102N / 06527W (Communication navigation surveillance/air traffic system)

A Provincial Aerospace Beech B300 (SPR700) from Wabush, NL (CYWK) to Halifax/Stanfield, NS (CYHZ) was observed off of the flight plan route and when the pilot was queried, they advised receiving direct routing from the previous controller. Direct routing was updated in the Canadian Automated Air Traffic System (CAATS) by Montreal (UL), but had not been received by Gander (QX) or Moncton (QM). No separation issues or impact to operations.

Incident at MONT-JOLI QC (CYYY)

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: A Pascan British Aerospace Jetstream Model (C-GPPS/PSC835) from Quebec/Jean Lesage, QC (CYQB) to Mont-Joli, QC (CYYY) and a Provincial Airlines Beech 1900D (C-FNCP/PVL903) from Halifax/Stanfield, NS (CYHZ) to Mont-Joli, QC (CYYY) estimated CYYY at the same time at 1259Z. PSC835 and PVL903 cancelled instruments. PSC835 proceeded for Runway 06. PVL903 proceeded for Runway 24 and had to slow down to allow PSC835 to re-enter Runway 06. The determined runway was Runway 24.

Incident at ÎLES-DE-LA-MADELEINE QC (CYGR)

A Jazz de Havilland DHC-8-402 (C-GGND/JZA8734) from Fredericton, NB (CYFC) to Iles-de-la-Madeleine, QC (CYGR) declared PAN PAN and reported flaps stuck at 15 degrees. The pilot requested diversion to Halifax/Stanfield, NS (CYHZ). The aircraft landed at 1758Z, No impact on operations.
UPDATE AOR-266121-V1: The Moncton (ZQM) air traffic control (ATC) advised the Charlottetown, PE (CYYG) flight service station (FSS) that a Jazz de Havilland DHC-8-402 (C-GGND/JZA8734) on an IFR flight from Fredericton, NB (CYFC) to Îles-de-la-Madeleine, QC (CYGR) declared "Pan Pan" on approach into CYGR. JZA8734 was now diverting to CYYG and the pilot requested the CYYG Aircraft Rescue and Fire Fighting (ARFF) unit be alerted and put into position. The aircrafts flap operation was abnormal. At 1718Z ZQM ATC advised that JZA8734 was now diverting to Halifax/Stanfield, NS (CYHZ) vice CYYG. At 1721Z ZQM ATC confirmed that the CYYG ARFF unit could "stand down". (Duplicate record AOR-266121-V1 / CADORS 2020A0491 deleted)
Update: TSB Report #A20A0033: C-GGND, a de Havilland DHC-8-402 aircraft operated by Jazz Aviation LP, was conducting flight JZA8734 from Fredericton Intl (CYFC), NB, to Îles-de-la-Madeleine (CYGR), QC. On approach to CYGR, the crew received a FLAP POWER caution indication and the flaps did not extend beyond 17 degrees. The crew declared PAN PAN with air traffic services (ATS) and requested a diversion to Halifax/Stanfield, (CYHZ), NS. The aircraft landed at CYHZ without further incident.

A WestJet Encore de Havilland DHC-8-402 (C-GKWE/WEN3485) on a flight from Halifax/Stanfield Intl, NS (CYHZ) to Ottawa/MacDonald-Cartier Intl, ON (CYOW) reported an emergency locator transmitter (ELT) signal, 4/5 on its descent from FL240 into CYOW. Signal became intermittent at 13 000 ft and stopped at 8 000 ft. The Joint Rescue Coordination Centre (JRCC) Trenton (TR) was advised. Various aircraft in the vicinity reported the ELT. Last positive report was received at 2135Z.

On touchdown on Runway 24R, a Westjet Boeing 737-6CT (C-GXWJ/WJA273) from Halifax/Stanifeld, NS (CYHZ) to Toronto/Lester B. Pearson, ON (CYYZ) experienced a bird strike. A vehicle (BMN73) reported finding a sparrow in pieces. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. WestJet flight crew of WJA273 reported a possible bird strike. WestJet maintenance performed a bird strike inspection in accordance with aircraft maintenance manual (AMM) 05-51-18 and LTV AMM 44-20-08. Evidence of bird strike was found on L1 window. No faults found. The aircraft was returned to service on August 16, 2020.

A Swoop Boeing 737-8CT (C-FLSF/WSW181) from Halifax/Stanfield, NS (CYHZ) to Hamilton, ON (CYHM) reported a bird strike landing Runway 30. Maintenance found remains on runway.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Bird strike was cleaned and the right-hand (R/H) main landing gear (MLG) inspection was carried out (C/O) in accordance with (IAW) aircraft maintenance manual (AMM) 05-51-18 and maintenance control manual (MCM) 10.4, with no faults found (NFF). The aircraft was deemed okay for service. No other evidence of a bird strike was found on aircraft.

Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

A weather cell was passing over the field when a Government Of Canada, Department Of National Defence Bombardier CL-600-2B16 (CFC3090) from Halifax/Stanfield, NS (CYHZ) to Ottawa/MacDonald-Cartier, ON (CYOW) started its approach towards CYOW. On final for Runway 07, the wind shear warning alarm sounded and CFC3090 elected to do a go around at 2040Z. The wind shifted to Runway 32 and CFC3090 tried an approach on this runway. They encountered a wind shear again and a did second go around at 2102Z. CFC3090 safely landed on Runway 32 at 2113Z.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (FOD (foreign object debris))

An Air Canada Boeing 777-333ER (C-FJZS/ACA606) from Toronto/Lester B. Pearson, ON (CYYZ) to Halifax/Stanfield, NS (CYHZ) was cleared for take-off from Runway 06L on a visual departure behind an Air Canada Embraer ERJ 190-100 IGW (C-FMZW/ACA1037) from Toronto/Lester B. Pearson, ON (CYYZ) to Denver, CO (KDEN). ACA606 aborted its take-off roll after observing that ACA1037 had aborted takeoff further down the runway due to a bird strike. ACA606 returned to the runway for takeoff. No further delays to the operation.
UPDATE Duplicate AOR 262592-V1: A red tailed hawk was found on the runway after ACA1037 advised of the bird strike. (Duplicate CADORS 2020O0469 / AOR 262592-V1 deleted)
UPDATE TSB Report #A20O0029: On 07 March 2020, at 0948 eastern daylight time, an Embraer 190-100 (E190) operated by Air Canada (registration C-FMZW) as flight ACA1037 was conducting a takeoff under visual departure procedures from Runway 06L. After the E190 began its takeoff roll, a Boeing 777-300 (B777), operated by Air Canada (registration C-FJZS) as flight ACA606, was instructed to position and hold onto Runway 06L. While the E190 was on the take-off roll, the Tower controller issued a takeoff clearance to the B777. Moments later, the E190 had a bird strike at approximately 135 knots. The E190 flight crew initiated a high-speed abort and made a radio call on the tower frequency that they were aborting the takeoff. As the E190 flight crew made their radio call, the B777 flight crew read back the takeoff clearance on the same frequency and commenced its takeoff roll. Shortly afterwards the B777 flight crew observed that the E190 was still on the runway and aborted the takeoff, reaching a maximum speed of about 110 knots. The B777 flight crew made a radio call on the tower frequency that they were also aborting the takeoff. Both aircraft exited the runway. The E190 returned to the terminal. The B777 waited on a taxiway for 45 minutes to allow the brakes to cool and then returned to the terminal. There were no injuries. There was no damage to either aircraft.

After it had received the instruction to fly direct AVVON, a Jazz Bombardier CL-600-2B19 (C-GNJA/JZA8548) from Ottawa/Macdonald-Cartier, ON (CYOW) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was seen 20 degrees off course on a heading of 094, and it came into conflict with a Jazz Bombardier CL-600-2D24 (C-GPJZ/JZA8633) from Halifax/Stanfield, NS (CYHZ) to Ottawa/MacDonald-Cartier, ON (CYOW). The air traffic services (ATS) unit instructed two aircraft to conduct a right turn to maintain the separation, which had been reduced to 1 000 ft and 3.8 NM. No impact on operations.

At 1540Z, a Government of Canada, Department of National Defence Airbus CC-150 Polaris (CFC4191) from Halifax/Stanfield, NS (CYHZ) to London/Gatwick, United Kingdom (EGKK) reported hearing an emergency locator transmitter (ELT) signal in the vicinity of Saint-Pierre, St. Pierre and Miquelon (LFVP), and continued hearing it for approximately 28 minutes until overhead of St. Johns, NL (CYYT), at which time the signal stopped. Other aircraft, as well as the ground facility at LFVP, were asked to monitor the frequency, though no further reports were received. No further impact on operations.

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3347) from Halifax/Stanfield, NS (CYHZ) to Toronto/Lester B. Pearson, ON (CYYZ), ready to depart from Runway 32, reported bird remains on the runway at the intersection of Taxiway Mike. The aircraft departed past that point. The Halifax International Airport Authority (HIAA) was advised, inspected the runway and found the remains of 1 hawk. One departure was delayed by 07 minutes and another departure opted to depart from Runway 23. No other impact.
