Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Smoke was observed from the left engine of a Sunwing Airlines Boeing 737-86J (C-GOWG/SWG9012) on departure from Montreal/Pierre Elliott Trudeau, QC (CYUL) for Hamilton, ON (CYHM). The pilot was advised by the terminal control unit (TCU). Twenty minutes later, a fuel spill was observed on Taxiway A up to Runway 24L. An inspection confirmed that the spill was from SWG9012 at the gate. The Montreal shift manager (GDQ) was notified. No impact to operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

At takeoff, there was a loss of separation for wake turbulence between an Air Canada Airbus A320-211 (C-FPWE/ACA2339) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Hamilton, ON (CYHM) and an Air Transat Airbus A330-342 (C-GKTS/TSC398) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Punta Cana, Dominican Republic (MDPC). Separation was reduced to 4.1 NM, while the standard was 5 NM. No other impact on operations.
UPDATE: TSB Report #A20Q0010: C-FPWE, an Air Canada Airbus A320 (ACA2339), was on an instrument flight rules (IFR) flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Hamilton/John C. Munro Int’l Airport, ON (CYHM). It conducted its takeoff from Runway 24L after C-GKTS, an Air Transat Airbus 330 (TSC398), which was on an IFR flight from CYUL to Punta Cana International Airport, Dominican Republic (MDPC). A loss of wake turbulence separation occurred during the initial climb, when ACA2339 was 4.1 nautical miles (NM) behind TSC398, even though the required separation was 5 NM. No wake turbulence was reported, and the 2 aircraft continued their flights to their respective destinations without incident.

An aircraft contacted the Hamilton, ON (CYHM) tower to advise that a Skylink Express Inc. Beech 1900C (C-GSKG/SLQ9742) from Hamilton, ON (CYHM) to New York/John F. Kennedy, NY (KJFK) that had departed Runway 12 was struck by a laser near Milford, ON. The exposure was reported as long and intentional. The crew continued with their flight. The Ontario Provincial Police (OPP) was notified.

The London, ON (CYXU) Flight Information Centre (FIC) received a call from the Hamilton, ON (CYHM) tower to close a VFR flight plan on an American, corporately registered Aerospatiale AS350 B3 (N125TG) on a transborder flight from West Mifflin, PA (KAGC) to Hamilton, ON (CYHM). The CYXU FIC had no information on the aircraft. Upon further investigation, FlightAware indicated that the helicopter had departed KAGC northbound to Hamilton. Leidos confirmed they also had no information on this flight.

Incident at HAMILTON ON (CYHM)

A CargoJet Airways Boeing 757-223 (CJT584) from Vancouver, BC (CYVR) to Hamilton, ON (CYHM), on downwind for CYHM, declared an emergency due to a flap issue. Aircraft circled airport to conduct system checks, then returned to land. Emergency equipment requested. Aircraft landed safely at 1739Z and taxied to gate. No further incident or impact to operations.
UPDATE TSB Report #A20O0034: Cargojet flight CJT584, a Boeing 757-200 (C-GCJT) aircraft, was on the downwind for Hamilton International Airport (CYHM), ON and declared emergency due to flap issue. The flight crew, upon selecting approach flap, reported receiving a “flap disagree” message and the flaps were indicating between 0 and 1. The flight circled airport to conduct system checks then returned to land. Emergency equipment was requested. The flight landed in YHM without further incident and was met by the ARFF upon arrival as a precaution. There were no injuries or damage. This aircraft had same problem on the previous flight going into Vancouver (CYVR) on 17 March 2020 (TSB A20P0037) and maintenance were unable to duplicate the snag on ground. The aircraft was taken out of service. A verification flight was mandated upon rectification prior to placing it back in service. The aircraft carried out the verification flight on 22 March 2020 with no faults noted. Multiple components were inspected and replaced as a precaution. Maintenance is continuing to investigate the issue.

The aircraft landed at CYHM and closed their flight plan, which the London Flight Information Centre (FIC) had never received a departure message about. A subsequent call to Leidos confirmed that the FP for the aircraft had never been activated.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence.The aircraft (C-FYBK) rejected its takeoff due to a Master Caution indication, related to an over wing exit "door not closed" indication. Troubleshooting revealed an accumulation of frost/ice which was inhibiting the proper operation of the door’s flight lock solenoid.

The flight of a Skylink Express aircraft (SLQ4738) from Hamilton, ON (CYHM) to Montreal/Mirabel Intl, QC (CYMX) was conducted by a Beech 1900C (medium), even though the flight data system (FDS) indicated a Cessna 208 (light). Impact: wake turbulence separation was not assured.
UPDATE from operator: The operator provided the flight plan details which confirmed the flight for SLQ4738 was filed for a Beech 1900. Incorrect input of aircraft type by air traffic control.

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX) (Aerodrome - visual aids)

At the Montreal/Mirabel, QC (CYMX) aerodrome, the light on Taxiway F was out of service (U/S). A Skylink Express Inc. Beech 1900C (SLQ550) from Hamilton, ON (CYHM) to Montreal/Mirabel, QC (CYMX) went onto F with no light. The light came on a few minutes later. After a fourth reset, the light was on. The airport duty manager (ADM) was notified.

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX) (ATM - inaccurate aeronautical information, ATM - weather observation systems, ATM - ILS irregularity)

At the Montreal/Mirabel, QC (CYMX) aerodrome, following an unknown problem that occurred around 0755Z, the MX10, MX12, and MX13 screens started to flash. There was interference in the various speakers. The mouse of MX10 and MX11 and the Human Weather Observation System (HWOS) were unserviceable (U/S). The instrument landing system (ILS) for runways 06 and 24 were U/S. Status light no longer working on the Monitor/Controller (MONCO) for localizer beacon (LOC) 24. Because the mouse for MX10 was U/S, the automatic terminal information service (ATIS) was also U/S. Weather observations were sent via the Quebec, QC (YQB) flight information centre (FIC) starting at 0800Z. The technical operations coordinator (TOC) and air traffic control (ATC) were notified. Due to poor weather conditions and the unavailability of the ILS on runways 06 and 24, a Skylink Express Beech 1900C (C-GSKU/SLQ550) from Hamilton, ON (CYHM) to Montreal/Mirabel, QC (CYMX) had to divert to Montreal/Trudeau, QC (CYUL) because the aircraft was incapable of an area navigation (RNAV).

Incident at ABBOTSFORD BC (CYXX)

A call was received from Pacific radio advising that a Kan-Arm Contracting Ltd. Piper PA-31 (C-GDJR) on a training flight from Chilliwack, BC (CYCW) was inbound to Abbotsford, BC (CYXX) with a total electrical failure and possibly no landing gear. The pilot gave their intentions and estimated time of arrival (ETA) at CYXX by telephone. Aircraft Rescue and Fire Fighting (ARFF) and 911 were put on standby. The aircraft landed safely with minimal operational impact. A Swoop Inc. Boeing 737-800 (WSW107) on a flight from Hamilton, ON (CYHM) to Abbotsford, BC (CYXX) was moved to a different approach and some VFR aircraft held out on the ground.
UPDATE TSB Report #A19P0167: C-GDJR, a privately operated Piper Navajo PA-31-310 aircraft took off from Chilliwack, (CYCW) BC on a training flight with two pilots on board. When the gear was selected up, the crew observed a gear unlocked indication. The gear was cycled and a gear down and safe indication was achieved. The crews intent was to return to CYCW, but when they selected flaps down there was no movement of the flaps and the landing gear down and locked lights faded out. The crew made the decision to divert to Abbotsford, (CYXX) BC because it had a long runway and emergency equipment. During the diversion to CYXX, all electrical power was lost and communications equipment failed. Using a cell phone, the crew contacted the Kamloops Flight Service Station and declared a Mayday. They explained the situation, stated their intention was to land in CYXX, and requested to have ARFF standing by. On short final, enough electrical power returned for the crew to see they had a landing gear down and locked indication. The aircraft landed uneventfully and taxied to the ramp. Maintenance determined via a continuity check that the RH alternator field wire had an intermittent fault caused by a broken wire and / or a bad terminal crimp. They also found the LH engine noise filter had been removed from the alternator circuit, and wire terminals were taped with automotive electrical tape.

Incident at Kelowna

TSB #A19P0066:C-FTOH, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG9903 from Kelowna (CYLW), BC to Hamilton (CYHM), ON with 6 crew members and 9 passengers on board. As the aircraft was taxiing out for the departure at CYLW, the flight crew was advised by the flight crew of a WestJet aircraft that fire was observed coming from the number 1 engine drain mast. The flight crew stopped the aircraft, and shut the number 1 engine down. ARFF came to inspect the aircraft and, after inspection, the aircraft was cleared to taxi back to the hangar for further inspection. The engine was inspected and a ground run was carried out. Pooled engine oil was determined to be the probable cause. The aircraft was released by maintenance and the flight was conducted to CYHM without further incident.

Incident at In the vicinity of: ABBOTSFORD BC (CYXX)

A Chinook Helicopters (1982) Diamond DA 42 (C-FSGN/REGS71) from Abbotsford, BC (CYXX) to Abbotsford, BC (CYXX) declared an emergency with Victoria, BC (YYJ) terminal control unit (TCU) around the HUH VHF omnidirectional range (VOR) area. REGS71 had shut down one engine due low oil pressure indication. A Swoop Inc Boeing 737-800 (WSW107) from Hamilton, ON (CYHM) to Abbotsford, BC (CYXX) that was on approach was pulled off and vectored behind REGS71 for Runway 07. REGS71 landed safely at 1713Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Maintenance inspection post incident revealed that the RH engine turbocharger had failed resulting in a loss of oil and oil pressure. The engine data downloaded and sent to Thielert for investigation who confirmed the turbocharger failure was recorded but no indications of any further issues. The oil was drained and a sample sent out for analysis. The oil analysis report was sent to Thielert which then allowed for engine return to service subject to further oil analysis monitoring. The turbocharger was replaced, ground run carried out and oil drained and inspected for any further indications of possible actions required. Oil inspection showed no sign of further contamination. The aircraft was returned to service.
Update TSB #A19P0052: C-FSGN, a Diamond DA 42 Twinstar aircraft operated by Chinook Helicopters, was conducting examination flight REGS71 from Abbotsford (CYXX), BC. During the flight in the vicinity of the Whatcom (HUH) VOR, the flight crew declared an emergency, and indicated that they had shut an engine down due to a low oil pressure indication. The aircraft returned to CYXX, and landed on Runway 07 without further incident. The operator’s maintenance determined that the turbocharger oil seal failed. The operator is working with Continental Engines affiliate to decipher error codes that were generated from the occurrence.

A Cargojet Airways Ltd. Boeing 767-323 (C-GXAJ/CJT302) from Vancouver, BC (CYVR) to Hamilton, ON (CYHM) rejected its takeoff from Runway 26L and taxied back to the apron. No operational impact.

While loading passengers, a Swoop Inc. Boeing 737-8CT (C-FONK/WSW106) from Abbotsford, BC (CYXX) to Hamilton, ON (CYHM) reported a medical issue with two passengers and advised requiring assistance.

Incident at ABBOTSFORD BC (CYXX)

The Abbotsford police called the Abbotsford, BC (CYXX) tower to request that a Swoop Inc. Boeing 737-800 (WSW106) from Abbotsford, BC (CYXX) to Hamilton, ON (CYHM) return to the apron. The aircraft was on Taxiway Alpha at the time. The aircraft returned to the apron and a police officer boarded. No impact to operations.
