Incident at ÎLES-DE-LA-MADELEINE QC (CYGR)

A Jazz de Havilland DHC-8-402 (C-GGND/JZA8734) from Fredericton, NB (CYFC) to Iles-de-la-Madeleine, QC (CYGR) declared PAN PAN and reported flaps stuck at 15 degrees. The pilot requested diversion to Halifax/Stanfield, NS (CYHZ). The aircraft landed at 1758Z, No impact on operations.
UPDATE AOR-266121-V1: The Moncton (ZQM) air traffic control (ATC) advised the Charlottetown, PE (CYYG) flight service station (FSS) that a Jazz de Havilland DHC-8-402 (C-GGND/JZA8734) on an IFR flight from Fredericton, NB (CYFC) to Îles-de-la-Madeleine, QC (CYGR) declared "Pan Pan" on approach into CYGR. JZA8734 was now diverting to CYYG and the pilot requested the CYYG Aircraft Rescue and Fire Fighting (ARFF) unit be alerted and put into position. The aircrafts flap operation was abnormal. At 1718Z ZQM ATC advised that JZA8734 was now diverting to Halifax/Stanfield, NS (CYHZ) vice CYYG. At 1721Z ZQM ATC confirmed that the CYYG ARFF unit could "stand down". Update: TSB Report #A20A0033: C-GGND, a de Havilland DHC-8-402 aircraft operated by Jazz Aviation LP, was conducting flight JZA8734 from Fredericton Intl (CYFC), NB, to Îles-de-la-Madeleine (CYGR), QC. On approach to CYGR, the crew received a FLAP POWER caution indication and the flaps did not extend beyond 17 degrees. The crew declared PAN PAN with air traffic services (ATS) and requested a diversion to Halifax/Stanfield, (CYHZ), NS. The aircraft landed at CYHZ without further incident.

An Air Canada Rouge Airbus A319-114 (C-FYIY/ROU1524) from Toronto/Lester B. Pearson, QC (CYYZ) to Fredericton, NB (CYFC) declared PAN PAN with the Boston Centre due to a hydraulic problem. The aircraft landed at Quebec/Jean Lesage, QC (CYQB) at 1538Z. Runway 24 was closed at CYQB until 1610Z.
UPDATE TSB Report # A20Q0028: C-FYIY, an Air Canada Rouge LP Airbus 319-114 operating as flight ROU1524, was conducting an instrument flight rules (IFR) flight from Toronto/Lester B. Pearson, ON (CYYZ) to Fredericton, NB (CYFC) with 129 passengers and 5 crew members on board. During cruise, when the aircraft was at 77NM southeast of Quebec, QC, the Electronic Centralized Aircraft Monitor(ECAM) generated a low hydraulic fluid message in the green system. The crew elected to divert to Quebec/Jean Lesage, QC (CYQB) and declared a state of emergency. The aircraft landed on Runway 24 without issue. There were no injuries or damage to the aircraft.

The controller tried establishing contact, but the aircraft was non-responsive. A Moncton Flight College Diamond DA 20-C1 (C-FNJW) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was instructed and read back to taxi via Delta and Bravo, and hold short of Runway 33. An Air Tractor AT-802 (TKR625) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was backtracking on Runway 33 for departure when the controller observed C-FNJW crossing the hold short line. The controller instructed C-FNJW to continue taxiing to line up for Runway 27. TKR625 was re-sequenced to number 2 for departure. Minor delay in getting TKR625 airborne for fire fighting.

While conducting the area navigation (RNAV) 27 approach, it was determined that a Moncton Flight College Piper PA-44-180 (C-FHFS) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) had a stuck microphone. The controller was able to have the aircraft squawk its identification to acknowledge receiving a clearance. The aircraft was given a light signal and landed safely at 1224Z. No impact on operations.

Aviation Incident Report: The Tower called the fire hall, because they had a report of what appeared to be a dead snake on Runway 27 at Fredericton, NB (CYFC), just past the hold short line West of the intersection. 727 responded with vehicle (Red 6) and found a dead snake along the side of the centre line. The snake was picked up and disposed of.

During this approach the aircraft entered Class E Gagetown, NB (CYCX) airspace. The CYCX tower called the CYFC tower to query what occurred. The pilot was notified and there was no impact to operations.

Incident at FREDERICTON INTERNATIONAL AIRPORT

Due to the winds changing, the tower controller at Fredericton, NB (CYFC) was forced to switch 3 solo students from Runway 27 to land on Runway 15. On departure from Runway 27, a Moncton Flight College Diamond DA 20-C1 (C-FJXD) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was instructed and read back to make a right turn and join the right downwind for Runway 15. The aircraft was subsequently told to turn base for Runway 15. C-FJXD was then observed to not have turned final and appeared to be joining a right downwind for Runway 27. A Moncton Flight College Diamond DA 20-C1 (C-FFQM) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was given an immediate right 360 and C-FJXD was given an immediate left 270 to join left base for Runway 15. C-FJXD landed safely on Runway 15 at 1700Z, without further incident.

The Fredericton International Airport Authority (FIAA) was advised and the aircraft was towed back to the flight school. Multiple Moncton Flight College (MFC) aircraft delayed their taxi due to the disabled aircraft.

Incident at FREDERICTON INTERNATIONAL AIRPORT (Aerodrome, runway or taxiway shutdown)

The pilot of a Moncton Flight College Beech C90A (C-FSXG) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) broke off their approach for Runway 27 to troubleshoot a potential landing gear problem. They returned for landing after all the indications pointed to the landing gear being down. C-FSXG advised that after landing they would be shutting down on the runway and requiring a tug to tow them back to the flight college. The Aircraft Rescue and Fire Fighting (ARFF) unit was on standby. The aircraft landed safely at 1214Z and was towed clear of the active runway at 1235Z. Minimal impact to the flight school aircraft operating at CYFC.
Update TSB Report #A20A0020: C-FSXG, a King Air C-90 operated by Moncton Fight College, was conducting a local training around Fredericton Intl Airport, NB (CYFC). While on approach, the crew conducted a missed approach to trouble shoot a landing gear issue. The aircraft returned and landed without incident, however shut down on the runway and was towed back to the flight school. Tower requested ARFF to be on standby. Maintenance found the hydraulic pressure was low and the 60 amp CB was popped. The power pack, 60 amp CB and 300 amp relay were replaced and the aircraft was returned to service.

Incident at FREDERICTON INTERNATIONAL AIRPORT (ATM - inaccurate aeronautical information, ATM - NAVAIDS/radar, ATM - weather observation systems)

The Fredericton, NB (CYFC) tower lost radar, Extended Computer Display System (EXCDS) and all weather information at 1448Z. The Technical Operations Coordinator (TOC) was advised. The equipment was operational again at 1519Z. TOC later informed that the uncoordinated Telecommunications company that was conducting work had caused the outage. There were minor delays and restrictions to flight school aircraft during the outage.

A 651406 NB Inc. Cessna 172N (C-GSFX) from Grand Falls, NB (CCK3) to Fredericton, NB (CYFC) entered the CYFC control zone without establishing two way radio communication. Upon investigation, it was noted that the aircraft appears to have also entered restricted airspace CYR724.

Incident at FREDERICTON INTERNATIONAL AIRPORT

A Moncton Flight College Diamond DV 20-C1 (C-FSXF) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was instructed to do a 360 in the circuit for spacing. During the 360, the aircraft drifted towards the final approach of Runway 15. After the 360, C-FSXF was sequenced to number 2 to follow a WestJet Encore Ltd. de Havilland DHC-8-402 (C-FENU/WEN3404) from Toronto/Lester B. Pearson, ON (CYYZ) to Fredericton, NB (CYFC) on the final for Runway 15. C-FSXF was then instructed to widen out their downwind due to being close to final approach. Traffic was then passed to WEN3404 and WEN3404 reported traffic in sight. WEN3404 then reported a resolution advisory (RA) and was in the missed approach. WEN3404 landed without further incident at 1703Z.

A Moncton Flight College Piper PA-44-180 (C-FSUQ) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) informed the tower that they may be having a left main landing gear issue. The pilot conducted a low approach and the tower informed the pilot that the gear appeared to be down. The Aircraft Rescue and Fire Fighting (ARFF) unit was placed on standby, the aircraft landed without further incident at 1448Z. A Moncton Flight College Diamond DV20 was delayed for approximately 15 minutes before landing.

Incident at SAINT JOHN NB (CYSJ)

A Sherob Management Ltd. Piper PA-31 (C-FRKG) from Fredericton Intl, NB (CYFC) to Saint John, NB (CYSJ) reported not having three green lights on final for Runway 32. The aircraft circled around to try to resolve the issue and then made a low pass on Runway 14. The flight service station (FSS) and two fire fighters in Aircraft Rescue and Fire Fighting (ARFF) trucks were able to observed 3 landing gears. The aircraft landed safely without any issues at 1739Z.
UPDATE Aviation Incident Report #16636: At 1720Z, the flight service station (FSS) called reporting an incoming Sherob Management Ltd. Piper PA-31 (C-FRKG) with a landing gear issue. A vehicle (Red3) took a standby position on the Vassey road and another vehicle (Red4) took up standby on the back road. The aircraft did a low fly-by to verify that the landing gear was in position for landing, as the pilot was getting no verification that the gear was locking into place. The aircraft landed on Runway 32 without incident, then was escorted to the Atlantic Flight Center by Red3 and Red4. The Aircraft Rescue and Fire Fighting (ARFF) unit returned to the hall.

Incident at FREDERICTON INTERNATIONAL AIRPORT (Aerodrome, runway or taxiway shutdown)

Upon landing, a Moncton Flight College Diamond DA 20-C1 (C-FSXF) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) indicated it had left the runway and was in the grass at 1905Z. The Aircraft Rescue and Fire Fighting (ARFF) unit was advised. Both runways were closed due to the aircraft being disabled in the runway protected area (RPA) of the Runway 09/27 and Runway 15/33 intersection. ARFF unit was advised. The aircraft was towed back to the flight school and the runways re-opened at 2014Z. Multiple flight school aircraft conducted low approaches while waiting for the aircraft to be moved and runways to be opened.
Update TSB Report #A20A0018: C-FSXF, a Diamond DA-20 aircraft operated by Moncton Flight College (MFC) was conducting circuits at Fredericton International Airport (CYFC), NB. The student pilot landed on Runway 27 at 1505 AST. The aircraft bounced and departed the prepared surface to the left side at approximately 30 degrees off the runway heading. The aircraft made contact with an edge light before coming to a stop approximately 50 feet off the side of the runway. Airport Rescue and Firefighting responded. The student pilot was uninjured and the aircraft received minimal non-structural damage. Both runways (09/27 and 15/33) were closed due to the proximity of the aircraft near the intersection. Several MFC aircraft remained airborne until the both runways reopened at 1614 AST.

The aircraft was queried and quickly corrected course to exit and continue their flight to CYSJ. A check fire was issued with range control. No impact on operations.

A Moncton Flight College Piper PA-44-180 (C-FZKG) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) reported an indication of their landing gear not being down. Aircraft conducted a low approach abeam the tower and also abeam the flight school to allow pilots/mechanics to observe. Gear appeared to be down. Incident at FREDERICTON INTERNATIONAL AIRPORT

A Moncton Flight College Diamond DA 20-C1 (C-FSXF) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was en route to training areas and requested to return to land due to rough running engine. The Aircraft Rescue and Fire Fighting (ARFF) unit was placed on standby and the aircraft landed without further incident.

Incident at 4NM N FREDERICTON INTERNATIONAL AIRPORT (CYFC)

TSB Report #A20A0011: C-FDSN, an Airbus A320 operated by Air Canada was conducting flight ACA680 from Toronto Lester B Pearson International Airport, ON (CYYZ) to St John’s International Airport, NL (CYYT). While in cruise at FL350, the number 2 engine rolled back to approximately 54% N1, while the number 1 engine remained at 92% N1. The crew descended to FL290 with auto thrust commanded to 85% and both engines responded. The flight continued to destination without further incident. Maintenance is investigating.

The Halifax Flight Information Centre (FIC) received the transborder PVFR (ICAO format) Flight Plan (FP) with an Estimated Time of Departure (ETD) of 1640Z for a Leading Edge Enterprises Ltd. Piper PA-31 (C-GKCN) from Portland International Jetport, OR (KPWM) to Fredericton, NB (CYFC). The departure (DEP) message was not received from the United States for flight plan activation. A call from London, ON (CYXU) FIC was received, informing they were closing the flight plan at 1815Z.

An Exploits Valley Air Services Beech 1900D (C-GLHO/EV780) from Fredericton, NB (CYFC) to Halifax/Stanfield, NS (CYHZ) pulled up when 4 miles final for Runway 05 due to an unstable approach and then returned to land on Runway 05 at 2007Z.

Multiple aircraft were flying VFR circuits when the weather quickly deteriorated to below VFR. The aircraft included a Moncton Flight College Diamond DA 20-C1 (C-FSXA) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC), a Moncton Flight College Diamond DA 20-C1 (C-FJZC) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC), a Moncton Flight College Diamond DA 20-C1 (C-FSXF) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC) and a Moncton Flight College Diamond DA 20-C1 (C-FFQM) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC). Visibility dropped to 1/2 mile in -SN and tower observed visibility was 1/4 mile. C-FSXA conducted an overshoot because they lost visual reference to the ground. All aircraft landed safely by 1528Z.

Controller observed a Moncton Flight College Diamond DA 20-C1 (C-FEGL) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) lining up for Runway 27 at Taxiway Alpha. C-FJXD was instructed to pull up and go around.
