The Gander Area Control Centre (ACC) reported that a U.S. Army Priority Air Transport Beech 200 (PAT993) from Reykjavik–Keflavik, Iceland (BIKF) to Narsarsuaq, Greenland (BGBW) descended without clearance from FL260 to FL150. Separation decreased to less than the required minimum with another U.S. Army Priority Air Transport Beech 200 Reykjavik–Keflavik, Iceland (PAT989) from (BIKF) to Narsarsuaq, Greenland (BGBW). No impact to operations.

Incident at 6100N / 04000W

An American, corporately registered Cessna P337H (N88TN) from Reykjavik, Iceland (BIRK) to Goose Bay, NL (CYYR) declared an emergency due to number two engine failure and advised diverting to Narsarsuaq, Greenland (BGBW). Helicopter dispatched from Greenland and escorted the flight into BGBW. Landed without incident at 1347Z. No traffic or impact to operations.

Incident at HOIST intersection

At 1651Z, an American, corporately registered Beech B300 (N60125) from Narsarsuaq, Greenland (BGBW) to Goose Bay, NL (CYYR) declared an emergency and requested priority for landing at CYYR. The aircraft was cleared direct and landed at 1743Z without further incident.
Update TSB Report #A19A0087: N60125, a privately operated Beechcraft B300 aircraft, was conducting a flight from Narsarsuaq airport (BGBW), Greenland to CFB Goose Bay (CYYR), NL with 2 crewmembers on board. When about 158 nm northeast of CYYR, the crew received an amber master caution low right fuel warning (R FUEL QTY) and observed that fuel was not transferring from the right wing tank to the right nacelle tank. The crew declared an emergency and landed about 52 minutes later without incident.

The flight was confirmed off course and after passing 6240N040W was re cleared original routing. No traffic.

An American corporately registered Douglas DC3 (N45366) from Narsarsuaq, Greenland (BGBW) to Reykjavík, Iceland (BIRK) entered controlled airspace at F130 instead of F110 as cleared by Nuuk flight information centre (FIC). The aircraft was immediately re cleared to F110 due to traffic with an American corporately registered Douglas C-47A (N103NA) from Narsarsuaq, Greenland (BGBW) to Reykjavík, Iceland (BIRK) at F130.
Update TSB #A19A0030: N45366, a privately operated Douglas DC3 aircraft, was conducting an IFR oceanic crossing flight from Narsarsuaq (BGBW), Greenland to Reykjavík (BIRK), Iceland. N103NA, a privately operated Douglas C-47B aircraft, was also flying the same route. At approximate position N62°/W040°, N45366 entered controlled airspace at 13 000 feet, instead of 11 000 feet as cleared by air traffic services. In doing so, a loss of separation with N103NA occurred. The required separation is 30 minutes in trail, while the actual separation was 18 minutes.

Accident at 5442N / 05821W

An American corporately registered Piper PA-46-350P (N757NY) from Goose Bay, NL (CYYR) to Narsarsuaq, Greenland (BGBW) crashed at position 5442N 5821W.
Update TSB #A19A0025: N757NY, a privately operated Piper PA 46-350P aircraft, had departed from Goose Bay (CYYR), NL on a VFR flight plan to Narsarsuaq (BGBW), Greenland with 2 pilots on board. At approximately 0824 ADT, the Joint Rescue Coordination Center (JRCC) detected an Emergency Locator Transmitter (ELT) signal from the aircraft. It was determined that the aircraft had crashed into a snow-covered hillside, approximately 31 nm north northeast of Rigolet, NL. Deteriorating weather conditions at the time were less than adequate for air search and rescue resources. Both occupants survived the initial impact, however, one individual later succumbed to their injuries. The survivor was transported to Makkovik, NL via snowmobile until the weather became suitable for an air transport to a hospital in Goose Bay. The individual sustained minor injuries, and was released from the hospital the following day. The TSB is investigating.

No IFR clearance had been issued for the descent. There was no traffic.

Flight was at F400, was cleared to F380 and was advised a clearance was not available below FL380. Flight then advised they were descending and proceeding VFR direct NA. Traffic was passed at 1152Z. No emergency declared.

Incident at 6200N / 04000W

A foreign registered Cirrus SR22 (A6CTR) and a foreign registered Cirrus SR22 (A6CTP) on flights from Narsarsuaq, Greenland (BGBW) to Reykjavík, Iceland (BIRK) entered controlled airspace at the same level, and not their cleared levels. Separation was re-established and the flights exited the Gander, NL (CZQX) oceanic control area (OCA) without further incident. Flights relayed that the altitude deviation was due to weather. Both flights were advised that a report would be filed.

An American corporately registered Cessna 525 (N858DP) from Narsarsuaq, Greenland (BGBW) to Goose Bay, NL (CYYR) was cleared off of not above (NA) to FL220 with opposite direction at FL230. N858DP levelled at FL220 and requested FL400. The aircraft then climbed without a clearance (CLR) and was seen climbing on ADSB. The aircraft was stopped at FL300. Traffic was PSK620 FL230 HOIST 56/55 58/50 5937/45 61/40 6207/35 63/30 EPENI ELDIS. The pilot for N858DP thought that they had received a CLR to FL320 and climbed without confirming it. The pilot said primary frequency reception was poor when they confirmed it on secondary they realized it was for FL220.

An American corporately registered Piper PA-32RT-300 (N32HF) on a VFR flight route from Reykjavík, Iceland (BIRK) to Narsarsuaq, Greenland (BGBW) at A040 ascended without an air traffic control (ATC) clearance to A105 at 62N038W at 1200Z. The aircraft advised it had flight planned the level change and was following the flight plan to ascend at 1200Z. The request to ascend was not in the flight plan. Nil Traffic. Flight was switched to Kangerlussuaq, Greenland (BGSF) at 1232Z.

An American corporately registered Piper PA-46-600TP (N600RU) from Narsarsuaq, Greenland (BGBW) to Reykjavík, Iceland (BIRK) was cleared on track, and route reported to be flown was 62/40 63/30 RK. When the aircraft entered the Reykjavik area of responsibility, the flight appeared on radar well south (approximately 120NM) south of cleared route, apparently routing towards 61N030W. All position reporting was done correctly as per cleared route.

Incident at In the vicinity of: 5940N 048W

At 1559Z, after passing 59N050W, an American corporately registered Cessna 421C (2740B) from Goose Bay, NL (CYYR) to Narsarsuaq, Greenland (BGBW) advised of an oil leak in the left engine. At 1615Z, the flight declared an emergency after losing the left engine and advised they had enough fuel to continue to destination. At 1622Z flight was given Kangerlussuaq, Greenland (BGSF) Information frequency 121.3. BGSF advised N2740B was unable to land BGBW due weather and eventually landed Paamiut, Greenland (BGPT) at 1813Z.
UPDATE TSB #A16A0107: N2740B, a Cessna 421 aircraft was en route from Goose Bay, Newfoundland and Labrador (CYYR) to Narsarsuaq, Greenland (BGBW) at flight level 210 when the pilot reported a loss of oil from the left engine. The flight was at 59N 048W when it lost engine power. The pilot shut down the engine and declared an emergency. The pilot continued on to Greenland but diverted to Paamiut airport (BGPT) due to poor weather at BGBW and landed safely.

Incident at In the vicinity of 6135N4253W (Communication navigation surveillance/air traffic system)

A United States Dep. of Defence Beech B200 (WING95) from Narsarsuaq, Greenland (BGBW) to Keflavik, Iceland, (BIKF) was questioned on their route and they stated that they were told to bypass the route of 62N40W and 63N30W. Gander Ocean confirmed with Sondrestrom that the flight had indeed been cleared over 62N40W 63N30W. No other traffic affected.

Incident at 120NM NE GOOSE BAY NL (YYR) VOR/DME

UPDATE: TSB#A16A0082: N690PK, a corporate Aero Commander 690A aeroplane, was en route at FL250 from Goose Bay, NL (CYYR) to Narsarsuaq, Greenland (BGBW). While in cruise flight, the aircraft sustained a complete and sudden loss of pressurization. The crew donned their oxygen masks, declared an emergency and returned to CYYR. The co-pilot, who is an FAA approved A and P aircraft engineer, inspected the aircraft and found that a hose had worked loose from the rear compartment environmental unit due to an improperly tightened clamp. The hose was reattached and the clamp was properly tightened. The repair was documented and the flight resumed to its destination.
At 1112Z, an American corporately registered Aero Commander 690A (N690PK) from Goose Bay, NL (CYYR) to Narsarsuaq, Greenland (BGBW) was 120 NE of Goose Bay, NL (YYR) VOR/DME and declared an emergency with pressurization problems. The flight was at FL250 and requested immediate descent and direct to CYYR. The flight was cleared as requested. At 1119Z, the flight was 90 miles east of YYR at 10,000 feet and cancelled the emergency. The flight continued to CYYR and landed without incident at 1147Z.

Air traffic control (ATC) requested other aircraft to attempt contact on 121.5 to find out if there was a problem. The aircraft advised of icing and wanted to land at the nearest airport. The aircraft landed at Makkovik, NL (CYFT) without incident at 2117Z.

20NM north of Narsarsuaq, Greenland (BGBW) a British Airways Boeing 747 400 (GBNLD / BAW5V) from London, UK (EGLL) to Vancouver, BC (CYVR) reported severe turbulence for a duration of 2-3 minutes with a speed deviation of plus/minus 25KTS and altitude deviation of plus/minus 400FT. Flight requested and was cleared to maintain M84. No other traffic effected.

An American corporately registered Cessna 510 from Goose Bay, NL (CYYT) to Narsarsuaq, Greenland (BGBW) was cleared in part ENNSO 59N050W at FL270. At approximately 1730Z when flight was expected at 59N050W the flight was observed on radar heading westbound at position 5703N05420W F285. When queried pilot stated he had turned around at 1712Z due to weather.

At 1429Z, an American corporately registered Lancair LC41-550FG (N209DW) from Narsarsuaq, Greenland (BGBW) to Reykjavik, Iceland (BIRK) reported passing 62N042W next 63N030W. Air Traffic Control (ATC) requested the pilot was routing 62N040W 63N030W which was confirmed at 1436Z. At 1448Z, the flight reported 6216N040W next 63N030W and at 1505Z reported 6224N03841W next 63N030W. The pilot advised of an auto pilot failure which put the aircraft in a right turn, auto pilot was reset but not before aircraft was off course.

At time 1244Z, an Air Greenland Beech 200 (OYPCL/ GRL811) from Narsarsuaq, Greenland (BGBW) to Nuuk, Greenland (BGGH) received a descent clearance from Gander international flight service station (IFSS) to descend below controlled airspace. The clearance had not been issued by Gander Oceanic and was instead an old clearance that had been issued to the flight approximately 2 hours earlier when the flight was inbound to BGBW. The flight accepted the clearance and descended from FL280. There was no traffic.

At 1720Z, Air Traffic Control (ATC) received a position report from an Air Iceland Fokker F27 (FXI466) from Narsarsuaq, Greenland (BGBW) to Reykjavik, Iceland (BIRK) over 62/40 headed to 63/30. The aircraft flight planned and was cleared 62/40 - 63/35. Kangerlussuaq Flight Information Centre (SF FIC) confirmed the flight was cleared over 63/35. At 1734Z, the flight was recleared direct 63/35. No traffic. No impact to normal operations.

At EVABU, An Air Greenland Boeing 737 700 (OYJTU/ GRL786) from Narsarsuaq, Greenland (BGBW) to Copenhagen, Denmark (EKCH) reported that their next point would be 63N40W when their oceanic clearance was 64N40W. The flight was queried and confirmed the incorrect point. Gander Oceanic then re-cleared the flight to the correct fix of 64N40W.

An American corporately registered Cirrus SR22 (N96FL) from Narsarsuaq, Greenland (BGBW) to Goose Bay, NL (CYYR) was coordinated by SF Flight Information Centre (FIC) at FL040 which is uncontrolled airspace. The flight passed 59N50W at FL100 which is still in uncontrolled airspace. At time 1845Z, the flight advised bearing 066 from HOIST at 10NM, which is controlled airspace FL055 - FL600. The flight reported at FL100, descending to FL060. Gander Air Traffic Control (ATC) made multiple attempts to contact the flight to confirm the flight level. There was no contact until in very high frequency (VHF) contact with Gander Low Level.
