Incident at OTTAWA / GATINEAU QC (CYND)

A Richcopter Inc. Cessna 150L (C-GACS) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) that was landing missed Taxiway A, ran onto the grass, and ended up getting back onto the taxiway a few dozen feet further.

Incident at VAL-D'OR QC (CYVO)

After reporting a problem with auto-pilot, a Nolinor Aviation Boeing 737-2B6C (C-GNLN/NRL955) from Val-d’Or, QC (CYVO), to Montréal/Mirabel, QC (CYMX) received and read back clearance for a visual approach for Runway 24 at CYMX. NRL955 was then seen crossing the approach path perpendicular to 4,500 ft and asked for a 270-degree left turn. The aircraft was guided to final and landed safely at 0157Z. The departure of a Morningstar Air Express Boeing 757-200 (MAL7054) on a flight from Montréal/Mirabel, QC (CYMX) was delayed by three minutes.

Upon initial contact with air traffic control (ATC), a Pascan British Aerospace Jetstream 3212 (PSC836) from Sept-Îles, QC (CYZV) to Baie-Comeau, QC (CYBC) was cleared to climb to 10,000 ft. PSC836 was observed climbing through 10,500 ft. When questioned, PSC836 stated it was climbing to 12,000 ft, despite having correctly read back the ATC clearance. No impact on operations.

Incident at 85NM ÉLÉONORE QC (CEL8)

An Air Creebec de Havilland DHC-8-101 (CRQ5664) from Élénore, QC (CEL8) to Rouyn-Noranda, QC (CYUY) was cleared to FL220. Correct read-back by the pilot. The air traffic controller (ATC) intervened at the same time that CRQ5664 went to FL230. No impact on operations.

On final for Runway 24R, a Sunwing Airlines Boeing 737-800 (SWG429) from Punta Cana, Dominican Republic (MDPC) to Montreal/Pierre-Elliott-Trudeau, QC (CYUL) asked to abort its approach due to flap problems. No emergency. SWG429 was instructed to fly the runway heading and climb to 3,000 ft. Despite a correct read-back, SWG429 deviated from the planned route and was observed climbing through 3,000 ft. A few minutes later, SWG429 reported having resolved its flap issues and landed without incident. No impact.

Incident at PRINCE GEORGE BC (CYXS)

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3285) from Vancouver, BC (CYVR) to Prince George, BC (CYXS) conducted an unplanned missed approach for Runway 33 at 2209Z. WEN3285 was switched over to the Vancouver Centre and was vectored back for the Instrument Landing System (ILS) 15. Aircraft landed without further incident.

Read back was given. C-GTCN started to taxi onto the runway. C-GTCN was instructed to stop. Another Cessna 172 that was 1NM final was instructed to pull up. C-GTCN was instructed to continue and take-off clearance was given when it was safe.

A frequency change was issued to a WestJet Boeing 737-800 (WJA657) from Toronto/Lester B. Pearson, ON (CYYZ) to Calgary, AB (CYYC) and it was read back by a WestJet Boeing 737-700 (WJA110) from Vancouver, BC (CYVR) to Calgary, AB (CYYC). The controller did not notice the readback was from the wrong aircraft.

Incident at LANGLEY REGIONAL BC (CYNJ)

An AAA Aviation Ltd. Cessna 172I (C-GUNE) from Langley, BC (CYNJ) to Langley, BC (CYNJ) landed on Runway 25 and was instructed to exit via Taxiway G and contact the ground controller. The aircraft started taxiing onto Taxiway G without contacting the ground controller. The controller reached out and issued an instruction to "taxi on Taxiway G and hold short of Runway 19". C-GUNE did not read back the hold short. The controller required multiple transmissions to get a read back.

Incident at NANAIMO AIRPORT (Incursion - runway - vehicle)

A truck (ST22) proceeded onto Runway 16/34 after being authorized to proceed via Taxiway C and hold short of Runway 16/34. ST22 read back the hold short instruction. An Alkan Air Beech 200 (AKN216T) from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) was 2NM final for Runway 34 and ST22 had to quickly exit the runway onto the grass to vacate the runway.

A Pacific Professional Flight Centre Ltd. Cessna 172P (C-GHFH) from Boundary Bay, BC (CZBB) to Boundary Bay, BC (CZBB) on a VFR flight plan (FP) reached the Information Request to Departure Station (QALQ) stage at 0600Z. Called the pilots cell phone and left 2 messages on voicemail. Checked CZBB Tower Aircraft Movement (TAM), with no indication of a departure (DEP) or arrival (ARR) from CZBB. Called the CZBB tower, they had no contact with the aircraft. Left message on voicemail for "Pro IFR". The aircraft reached the information request (INREQ) stage at 0630Z. Joint Rescue Coordination Centre (JRCC) Victoria called and advised of details. Was in the process of getting Delta Police to arrange a ramp check when the pilot called back. The pilot indicated they never did depart from CZBB, but forgot to call back to cancel their FP. FP closed. Called JRCC Victoria back to advise communication search terminated.

Incident at Kelowna

A Penticton Flying Club-COPA Flight 50 Cessna 172L (C-GBFB) from Penticton, BC (CYYF) to Kelowna, BC (CYLW) was inbound from the southwest (SW) and was told to fly northbound over the eastern shore of Okanagan Lake. The tower cleared the aircraft for the left downwind for Runway 34, however, C-GBFB read back "right downwind". Tower said "negative", cleared the aircraft for the left downwind, and the readback was correct. C-GBFB crossed the midfield and joined the right downwind.

The Technology Operations Coordinator (TOC) for Vancouver (YVR) was called to request the status of the NOTAM for the Hope, BC (CYHE) remote communications outlet (RCO). The TOC called back at 0005Z to say they called the Flight Information Centre (FIC) at 1814Z to cancel the NOTAM. The NOTAM had not been cancelled, but was at 0005Z.

A privately registered Piper PA-28-151 on a flight to Williams Lake, BC (CYWL) exited Runway 12/30 onto Taxiway B, which does not have winter maintenance. A vehicle (ST21) assisted the aircraft to get back onto Runway 12/30. The aircraft continued on their flight plan after a delay of approximately 15 minutes. No other impact to operations.

A Horizon Air Industries Inc. de Havilland DHC-8-402 (N403QX/QXE2517) from Kelowna, BC (CYLW) to Seattle/Tacoma, WA (KSEA) was cleared, from Apron 1, to taxi via Taxiways C and D, and instructed to hold short of A. Hold short instruction was read back. The aircraft subsequently taxied onto A.

Incident at VICTORIA INTL BC (CYYJ)

The pilot of a Pacific Coast Jet Charter Inc. Cessna 525A (N920JR/PXT920) from Victoria, BC (CYYJ) to Los Angeles, CA (KLAX) filed duplicate flight plans with different routings under both the aircraft identification (ACID) and civil registration. The tower cleared the pilot via the flight plan route for PXT920, and the pilot read back the clearance. When the departure controller cleared PXT920 to the first point on route of flight, the pilot queried the clearance. At this point it became clear there were two different routes and the pilot was re-cleared appropriately. No other operational impact.

While in position for departure, a Jazz de Havilland DHC-8-402 (C-GJZR/JZA8413) from Penticton, BC (CYYF) to Vancouver, BC (CYVR) reported a passenger experiencing a medical issue. No emergency was declared and no assistance was required at the time, but the aircraft requested to stay in position on the runway if traffic allows. The aircraft subsequently taxied back to the terminal and had the airline operations call an ambulance. Area control centre (ACC), traffic management unit (TMU), airport manager (APM) and airport operations advised. No impact to operations.

A Canadian Flight Centre Cessna 172R (C-GZPK) from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK) departed CYPK at 0646Z on a return flight plan (FP) back to CYPK, with an estimated time of arrival (ETA) of 0746Z. After a communications search, the Royal Canadian Mounted Police (RCMP) was dispatched to CYPK for a ramp check, where the aircraft was located at 0943Z. The FP was closed.
