Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX)

TSB Report #A20Q0006: C-GNLN, a Nolinor Inc. B7327-200 (NRL942), was on an IFR flight from Montreal/Mirabel Intl., QC (CYMX) to Val-d’Or, QC (CYVO) with 6 crew members and 61 passengers on board. When the landing gear was raised after takeoff, the crew received a nose wheel disagree indication. The crew declared an emergency and completed the checklist in the Quick Reference Handbook (QRH). When the gear was lowered, all the wheels were down and with normal indications. The aircraft went back and landed on Runway 06 at CYMX with Aircraft Rescue and Fire Fighting (ARFF) services on alert. The aircraft landed without incident, there were no injuries, and the aircraft was not damaged.

Incident at VAL-D'OR QC (CYVO)

TSB Report A20Q0001: C-GSPW, an Air Inuit Boeing 737-200 (AIE752) with 4 crew members and 28 passengers on board, was on an IFR flight from Montreal Pierre Elliot Trudeau Intl, QC (CYUL) to Val-dOr, QC (CYVO). Following the landing at CYVO, the flight crew received a fire indication from the auxiliary power unit (APU) as well as an indication that the fire extinguishing bottle had discharged. Noticing that the fire indication was still present, the flight crew brought the aircraft to a stop on the runway and requested the presence of Aircraft Rescue and Fire Fighting (ARFF) services in case of an accident so that it could verify whether smoke or flames were present at the back of the aircraft. The aircraft, having not received any confirmation of the presence of smoke or flames, taxied to its designated parking area and proceeded with the quick unloading of passengers since the fire indication was still present. All occupants evacuated the aircraft. ARFF personnel arrived a few minutes later. When the APU was checked, it was discovered that there was no trace of fire. No one was injured and the aircraft was not damaged.

A Government of Quebec Canadair CL600-2B16 (series 601-3R) (C-GURG/QUE10) from Mont-Joli, QC (CYYY) to Quebec/Jean Lesage, QC (CYQB) conducted a missed approach on Runway 24 due to an instrument problem. While taking off from Runway 06R, an Endeavor Air Bombardier CL-600-2D24 (Series 900) (EDV5526), from Montreal/Trudeau, QC (CYUL) to Atlanta/Hartsfield-Jackson, GA (KATL) reported problems with its main landing gear and declared an emergency. Coordination carried out. EDV5526 was on the ground on Runway 24L at 2209Z. Impact: Runway 06R/24L was closed from 2209Z to 2241Z. The National Operations Centre (NOC) and the Aviation Operations Centre (AVOPS) were notified.
UPDATE: TSB Report #A20Q0008: N695CA, an Endeavor Air CL-600-2D24 (CRJ900) (EDV5526), was on an IFR flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Hartsfield/Jackson Atlanta Intl., GA (KATL) in the U.S. with 76 passengers and 4 crew members on board. During the initial climb after takeoff from Runway 06R, a brakes overheated indication appeared. The crew declared an emergency and went back to land on Runway 24L without incident, and afterwards, the aircraft was towed to the gate. No one was injured, and the aircraft was not damaged. The post-flight inspect revealed that the indication was erroneous.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Jazz de Havilland DHC-8-311 (C-FSOU/JZA8684) from Montreal/Trudeau, QC (CYUL) to Bagotville, QC (CYBG) lost a wheel from its left interior back gear at takeoff from Runway 24L. JZA8684 burned off fuel and declared an emergency at 1933Z. JZA8684 landed without incident at 2014Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During initial climb, the flight crew was informed by the air traffic controller (ATC) and the flight attendant (FA) that the aircraft (C-FSOU) lost the left inboard (#2) main wheel assembly. The decision was taken to execute an air turn back after burning some fuel. An emergency was declared and the aircraft landed without further incident. The aircraft stopped on the taxiway, where passengers were deplaned and bused to the terminal. Troubleshooting determined the main wheel bearings failed, allowing the assembly to separate from the gear axle on rotation. As the axle sustained some damage, the landing gear assembly along with the brake units and wheel assemblies were replaced before returning the aircraft to service. The main wheel assembly has been recovered. On January 4, 2020, another aircraft (C-GABO) suffered a No. 3 main wheel bearing failure while taxiing to the gate. Due to the failures on these aircraft, a fleet campaign was commenced on the DHC classic fleet (All DHC-8-102/301/311) to inspect and report on the condition of the main wheel bearings. A Safety Management System (SMS) investigation is currently underway. Further updates on the results of the campaign and SMS investigation will be made in Continuing Airworthiness Web Information System (CAWIS) as part of the follow-up on the two Service Difficulty Reports (SDRs) submitted by the operator.

While passing 11 000 ft, and at the moment of frequency transfer to Boston centre, an Envoy Air Embraer E135 (ENY3989) from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to New York/La Guardia, NY (KLGA) declared an emergency due to a pressurization problem, then went back down to 10 000 ft. ENY3989 requested to continue en route. Coordination carried out. No impact. The National Operations Centre (NOC) and the Aviation Operations Centre (AVOPS) were notified.
UPDATE: TSB Report #A20Q0002: N824AE, an Envoy Air Inc. Embraer 135 (ENY3989), was on an IFR flight from Montreal Pierre Elliott Trudeau, QC (CYUL) to LaGuardia, NY (KLGA) in the U.S. with 44 passengers and 3 crew members. At approximately 18 NM southwest of CYUL, while the aircraft passed 11 000 ft on climb, the crew declared a state of emergency due to a pressurization problem and immediately went back down to 10 000 feet. The crew advised ATC that the situation was stable and that the aircraft was continuing the flight to its destination, KLGA.
