Beech 1900D (NDL191) from Plattsburgh, NY (KPBG) to Montréal/St-Hubert, QC (CYHU) that was instructed by the terminal control unit (TCU) to intercept the final approach at 6 NM was observed on base at 3 NM from the threshold. NDL191 then returned downwind to once again intercept the final at 6 NM after descending to 2,600 ft and climbing back to 3,000 ft before getting back on final. No other impact.

The Air Traffic Service (ATS) unit advised the aircraft to "turn course 060". The separation between AIE781 and a Jazz de Havilland DHC-8-402 (C-GYJZ/JZA8715), from Québec/Jean-Lesage, QC (CYQB) to Montréal/Pierre-Elliott Trudeau, QC (CYUL) was reduced to 2.6 NM. Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX) (Aerodrome, runway or taxiway shutdown)

A Les Investissements Nolinor Cessna 172L (C-GHWK) from Montréal/Mirabel, QC (CYMX) to Montréal/Mirabel, QC (CYMX) reported that its brakes were seized short of Runway 06 on K. Airport manager advised. Unable to clear through K and part of B was closed due to work. A Nolinor Aviation Boeing 737-33A (C-GNLQ/NRL943) from Val-dOr, QC (CYVO) to Montréal/Mirabel, QC (CYMX) had to clear the runway by B5 and wait on Taxiway 24 platform for approximately 10 minutes before getting back on the runway and clearing by K, after C-GHWK was towed to Taxiway Holding Platform 06.

There was a telecommunications link outage impacting The Extended Computer Display System (EXCDS), weather data entry (WDE), NAV CANADA Auxiliary Radar Display System (NARDS), air traffic controller (ATC) 445, and Val-d’Or, QC (CYVO) 118.5 MHz. The Technology Operations Coordinator (TOC) was informed, Bell link was out of service (U/S). The CYVO remote aerodrome advisory service (RAAS) was not provided. Came back online on September 12 at 1600Z. The Québec City/Jean Lesage Int’l, QC (CYQB), ATC, Roberval (CYRJ) observatory was informed.

An American, privately registered Maule M-7-235 from Rivière-du-Loup, QC (CYRI) to Sept-Îles, QC (CYZV) landed on Runway 27, and when it tried to make a U-turn, the aircraft ended up completely outside the runway lights. The aircraft performed a few manoeuvres in the grass before rolling back onto the runway and exiting at Delta. Impact: delay to an inbound Exact Air inc. Beech A100 (ET828) from Chicoutimi/Saint-Honoré, QC (CYRC) to Sept-Îles, QC (CYZV), which had to extend downwind and conduct a 360° on final.

Incident at SEPT-ÎLES QC (CYZV)

TSB Report #A21Q0047: A Les Hélicoptères Canadiens Airbus AS355N helicopter (C-FXIH) was on a visual flight rules flight from Sept-îles, QC (CYZV) to the IOC helipad at approximately 4 NM south-west. Shortly after take-off, the red “REG” warning light lit up and extinguished at the same time as the Ng fluctuations (engine gas generator) appeared. An intermittent aural warning sounded, indicating rotor revolution overspeed (RPM); however, the pilot noticed that the RPM were in the normal functional range. Following this, the “ENG P” and “R/H GEN” warning lights lit up and the “delta Ng” indicator of engine number 2 squawked zero. The pilot followed procedures for continuing the flight with only one functioning engine. The aircraft turned back and landed without issue on Taxiway Bravo at CYZV. Maintenance crew found that the digital engine control unit (DECU) had reacted to a faulty overspeed warning and had cut the fuel feed on engine number 2.

TSB Report #A21Q0031: C-GJZX, a de Havilland DHC-8-402 operated by Jazz under the callsign JZA8975, was about to perform an instrument flight rules flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Ottawa/Macdonald-Cartier, ON (CYOW) with 5 passengers and 4 crew members. While pushing back and starting the engines, a flight crew member became physically incapacitated. The aircraft returned to the boarding area and the crew member was replaced. There were no injuries or damage to the aircraft.

Aviation Incident Report #16964: At 1415Z, a Northern Air Charter Cessna 208 (C-FNME) from Natashquan, QC (CYNA) to La Tabatière, QC (CTU5) landed on Runway 05 at CTU5. After landing and turning around to taxi back to the apron, the aircraft sustained a flat tire on the right side of the aircraft. The aircraft had to be towed back to the apron. There was no damage to the runway.

Incident at BAGOTVILLE QC (CYBG)

Aviation Incident Report #16596: A Provincial Airlines Beech 1900D (C-FNCP/PVL1934) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Bagotville, QC (CYBG) was inbound on the area navigation (RNAV) approach for Runway 11. A Pascan British Aerospace Jetstream Model 3212 (C-GPPS/PSC837) on a flight from Bagotville, QC (CYBG) was advised of the inbound traffic while taxiing, when traffic was 10NM final. While taxiing, PSC837 was instructed to take off when PVL1934 was 4.7NM final, and PSC837 was given an immediate take off clearance at its discretion (flight advisory in effect). PSC837 read back the clearance when PVL1934 was 4.1NM final. When PVL1934 was 2.9NM from the threshold, the pilot asked if they were cleared to land, at which point the Flight Advisor cancelled the take off instruction for PSC837. Having received no response from PSC837, the Flight Advisor repeated that the take off instruction was cancelled (while PVL1934 was 2.3NM final). The pilot of PSC837 responded that they were rolling and proceeded to take off while PVL1934 was 2.1NM final. PSC837 cleared the intersection of Runway 11 when PVL1934 was 1NM from the threshold of Runway 11 and 2.1NM from the intersection of Runway 11 and Runway 18. Once PSC837 was clear of Runway 11, PVL1934 was given its landing instruction and landed without further incident.
