Incident at KAMLOOPS BC (CYKA) (ATM - inaccurate aeronautical information)
The pilot of a Skylink Express Inc. Cessna 208B (SLQ702) from Vancouver Intl, BC (CYVR) to Victoria Intl, BC (CYYJ) contacted the Kamloops (YKA) flight information centre (FIC) to enquire about the NOTAM function on the Aviation Weather Web Site (AWWS), as they were unable to get NOTAMs on the website. A quick test indicated that the NOTAM function on the website was unserviceable (U/S). NAV Canada (NC) customer service was advised.

Incident at LONDON FIC / FIC DE LONDON (ATM - weather observation systems)
At 0800Z, the Canadian Meteorological Aviation Centre (CMAC-E) called and informed the London Flight Information Centre (FIC) that they were unable to issue significant meteorological information (SIGMET) due to a problem on their end. SIGMET was not available on any of the following operational systems: the Flight Service Station Information Management System (FIMS), the Collaborative Flight Planning Services (CFPS), or the Aviation Weather Web Site (AWWS). CMAC agreed to fax SIGMET to the London FIC in the interim until the problem is rectified. By 1505Z, no updated SIGMET was received from CMAC. The National Operations Centre (NOC) was contacted and they agreed to fax London FIC updates. The Winnipeg (CZWG) Flight Information Region (FIR) SIGMET A3, valid at 1345Z, was received at 1506Z and broadcast 1 hour and 50 minutes late. Operational impact: SIGMET was not readily available to weather briefers to provide flight safety information to pilots on the phone or radio. Mitigation: London is copying faxed SIGMET from the NOC for specialists in each Ontario sector.

Incident at DAWSON CREEK BC (CYDQ)
At 0022Z a Mile Zero Flying Club Cessna 172M (C-GSSZ) from Dawson Creek, BC (CYDQ), was observed on air traffic service (ATS) Surveillance entering the CYDQ control zone at 3400' ASL 5NM west northwest. The aircraft appeared to join a left downwind for Runway 24, descended to 2900' and then disappeared off surveillance. Flight service station (FSS) attempted to contact the aircraft numerous times on 122.20. No response was heard. An aircraft was observed taxing south on Taxiway alpha at 0030Z via AWWS CYDQ weather camera. Impact on operations: Advisory service not provided.

Incident at NATIONAL (Canada) (ATM - inaccurate aeronautical information)
At 1305Z, a major weather information outage occurred and lasted until 25/0102Z. All weather information available via Flight Information Management System (FIMS), Flight Service Station Weather Graphics System (FWGS), Multipurpose Information Display System (MIDS), Automatic Data Acquisition and Processing System (ADAPS) and Aviation Weather Web Site (AWWS) unavailable during this period. Operational impact: preflight briefings and inflight services were provided using external websites; increased workload due to unavailability of AWWS.

Incident at PRINCE RUPERT BC (CYPR)
Privately N registered Piper PA18; FSS had a written flight strip in the proposed section for the aircraft and after looking at the Automatic Weather System (AWWS) weather camera of the ramp at Prince Rupert (CYPR) were unable to see it. After checking the station log and Nav Canada Aircraft Movement System (NCAMS) it was determined there was no record of the aircraft departing. Flight Service Station (FSS) checked with the airport staff. It was not there and had departed between 120000Z and 120100Z (afternoon of May 11, 2011). The airport staff could not give an exact time.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB) (ATM - other)
Il y a eu perte de la réception des produits météorologiques, alpha-numériques et cartes graphiques, sur les systèmes de gestion de l'information des FSS (FIMS) et de graphique météorologique des stations d'information de vol (FWGS), sur le réseau national. Le coordonnateur des opérations techniques (TOC) a été avisé et il a mentionné que les techniciens de jour seront avisés. Cet événement a causé un ralentissement de service au Centre d'information de vol (FIC) de Québec car les informations pour les exposés météorologiques ont du être obtenues via le site web de la météorologie à l'aviation (AWWS) de Nav Canada. Le retour graduel du réseau a eu lieu vers 1300Z.*** ** *** The was a receive loss of the alphanumeric , graphics maps and weather products on the flight service stations (FSS) management systems (FIMS) and FSS weather graphics (FWGS) on the national network. The technical operations coordinator (TOC) was advised, and he/she indicated that the day shift technicians will be notified. This occurrence caused a service slowdown of the Quebec City flight information centre (FIC) since the weather briefing information had to be obtained from the Nav Canada aviation weather website (AWWS). The network was gradually restored around 1300Z.

NAV CANADA staff at Edmonton F.I.C. reported that due to a power failure in Ottawa, no weather was being disseminated on AFTN to either FWGS, FIMS or the AWWS website. No current weather could be passed for FISE purposes or during weather briefings as all data was old. Ops. impact -- unknown.

NAV CANADA staff at London F.I.C. reported that the Aviation Weather Web Site (AWWS) underwent scheduled maintenance from 1100Z to 1400Z. During 1230Z to 1320Z, the AWWS was unserviceable. Ops. impact -- unknown.

Incident at EDMONTON INTL AB (CYEG) (ATM - other)
Edmonton FIC reported that some time between 2300z and 0000z the weather circuit failed. All current weather for NAV CANADA facilities in Northern Alberta, Saskatchewan and Manitoba was not available on MIDS, FWGS or AWWS website. Briefers were unable to provide up to date weather and altimeters to pilots during briefings or FISE contacts. The system was restored at 0200z.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (ATM - other)
The Halifax FIC FWGS server became unserviceable. No products were available at AWBS positions. AWWS site was being used for AWBS. Impact was minimal as products were being received from the backup system. There was no product looping or zoom in/out capability for RADAR and Satellite imagery. FWGS was returned to service at 1820Z.
