A vehicle (SU926) asked to drive from Taxiway AL to the loading rack. The aircraft was authorized to taxi on Taxiways A, H and F. SU926 was seen driving westward on Taxiway J. When asked, it confirmed having used the wrong taxiway and said that it was leaving from Taxiway T. It received new clearance to drive on Taxiways J and F.

A Nolinor Aviation Boeing 737-400 (NRL665) from Bagotville, QC (CYBG) to Quebec/Jean Lesage, QC (CYQB) was cleared for the area navigation (RNAV) Z Runway 24 approach, according to the minimum vectoring altitude (MVAS). The crew did not descend. When the aircraft was established on final, they requested to pull up and conduct a go-around.

A Piedmont Airlines Embraer EMB-145LR (N638AE/PDT6120) from Philadelphia International Airport, PA (KPHL) to Québec/Jean-Lesage Int’l, QC (CYQB) was cleared to perform an area navigation (RNAV) Z approach for Runway 29 at CYQB. At MAXOG, instead of turning left to establish on final, the aircraft turned right. The air traffic controller (ATC) brought it back to conduct a visual approach for Runway 29. Approach clearance for a Republic Airways Embraer 170-200LR (N729YX/RPA3588) from Newark Int’l, NJ (KEWR) to Québec/Jean-Lesage Int’l, QC (CYQB) was cancelled. There was also a slight delay in finally obtaining clearance to conduct a visual approach for Runway 29.

A PAL Airlines de Havilland DHC-8-402 (PVL1909) from Mont-Joli, QC (CYYY) to Québec/Jean-Lesage, QC (CYQB) landed and then took off again even though Runway 06/24 was closed by NOTAM. Movements approved by the airport authority. A vehicle (ST23) ensured that no one would enter the manoeuvring area before PVL1909’s takeoff.
UPDATE: AOR 291820-V3: A PAL Airlines de Havilland DHC-8-402 (PVL1909) landed, then took off again from Runway 06/24. During the manoeuvres, the aircraft taxied on a portion of Taxiways A and B, which were closed by NOTAM. Movements approved by airport management. A vehicle (ST23) ensured that no one went on the manoeuvring area before takeoff.

After receiving and reading back instructions to fly direct OMAXU and climb through 4,000 ft during pull-up on first approach to Rouyn-Noranda, QC (CYUY), a Propair Inc. Beech A100 (C-GDPI/PRO100) from Val-d’Or, QC (CYVO) to Rouyn-Noranda, QC (CYUY) was observed by CYUY radio landing after having conducted a second approach (area navigation [RNAV] approach for Runway 08 via OMAXU) without clearance for the second approach. Strong and clear emergency locator transmitter (ELT) signal heard for approximately 45 seconds. A Richcopter Inc. Robinson R44 (C-FOLY), on the ground at Ottawa/Gatineau, QC (CYND), contacted the air traffic service (ATS) unit to let them know the signal was coming from them, a helicopter. Area control centre (ACC) notified. No impact on operations.

Accident at 5038N / 07129W

Crash of a privately registered, amateur-built Wagaero Sportsman 2+2 signaled by emergency locator transmitter (ELT) north of Chicoutimi. Joint Rescue Coordination Centre (JRCC) Trenton on site. National Operations Centre (NOC), Transportation Safety Board of Canada (TSB), Sûreté du Québec (SQ), and Aviation Operations Centre (AOC) were notified and active. No further impact on operations.
UPDATE: TSB #A22Q0116: An amateur-built Wagaero Sportman 2+2 on floats was on a visual flight rules (VFR) flight in the Lac Onistagane, QC region (about 16 nautical miles south of Alma, QC (CYTF). The Canadian Mission Control Centre captured a distress signal sent out by the emergency locator transmitter (ELT) and the aircraft was found, upside down, in a woody and isolated area in the Lac Onistagane region. The 2 occupants were fatally wounded. Inspection of the wreckage at the accident site revealed that the left wing had detached in flight.

Pearson Intl, ON (CYYZ), westbound at FL360, reported hearing an emergency locator transmitter (ELT) signal in the vicinity of Sept-Îles (CYZV), QC. The signal started suddenly at 2235Z and faded out by 2237Z. Other aircraft asked to keep a listening watch in the area. Joint Rescue Coordination Centre (JRCC) Trenton advised.

After asking questions, the airport duty manager (ADM) confirmed that the work was not finished. Taxiway I was available at 1945Z.

After being instructed to climb to an altitude of 3,000 ft, a privately registered Cessna 185D on floats, on a flight from Québec/Lac St-Augustin, QC (Water) (CSN8), was observed descending into the Québec bridge area. Shortly afterwards, the aircraft declared an emergency and told the the air traffic controller (ATC) that it was going to attempt an emergency water landing on the Saint Lawrence River. The aircraft landed safely on the water at 1543Z and was towed away by onsite emergency services. No impact on operations .

On Taxiway Alpha, an Orizon Aviation Québec Inc. Cessna 152 (C-GJBF) from Québec/Jean-Lesage, QC (CYQB) to Québec/Jean-Lesage, QC (CYQB) was instructed to taxi on taxiway Delta1 to reach the apron. C-GJBF turned on Taxiway Delta then received new instructions to taxi on Delta, Bravo and Bravo1. The aircraft taxied on Taxiway B2. No impact.

A privately registered Piper PA-28-161 from St-Jean, QC (CYJN) to Bromont, QC (CZBM) was observed backtracking without clearance on Taxiway D and Runway 29 and exiting the runway at the Runway 02 threshold run-up area. The aircraft signaled its intention, but did not answer the many calls from the air traffic service (ATS) unit. Radio contact was only established once the aircraft was at its run-up bay (at 1235Z). No impact on operations.
