Incident at 4507N / 07327W

The Royal Canadian Mounted Police (RCMP) contacted the Montreal area control centre (CZUL) shift manager to advise that a privately operated helicopter, burgundy in colour, was flying over Highway 15 at an altitude of 100 meters, near Saint-Bernard-de-Lacolle, and that it was heading Northeast (NE). No aircraft was identified on radar in this sector. No impact on operations.

Incident at 4527N / 07252W (ATM - inaccurate aeronautical information)

An official of the "Association Québécoise de vol libre", who is responsible for the restricted airspace CYA621 (H) in Mont Yamaska, QC, contacted the Quebec flight information centre (FIC) to validate if their free flight zone was active, because an aircraft was flying at low altitude in their departure trajectory. After verification, the Designated Airspace Handbook (DAH) stipulates that the CYA is active during "Cont daylight". However, there is no activation schedule on the VFR navigation chart (VNC) AIR5002, and the zone is indicated as active from May 01 to October 31 during the day and overtime (O/T) by NOTAM on the VFR terminal area chart (VTA) AIR1903. The manager therefore requested that a NOTAM be issued to clarify the situation. The NOTAM was issued under the N3529/20. Operational impact: The operations of the free flight club were disrupted and a dangerous situation arose due to the presence of an aircraft on the departure path of the hang gliders and the proximity of 2 types of devices in the same space.

The officer advised that they would assess the situation before issuing a NOTAM because they did not know how long it would take. The Area Control Centre (ACC) and Air Inuit dispatch were informed.

Incident at 16NM SW MONTRÉAL INTL (MIRABEL) QC (CYMX)

An Académie Aéronautique Cessna 172N (C-GIGW) from Montreal/Mirabel, QC (CYMX) to Montreal/Mirabel, QC (CYMX) had radio problems at 16NM Southwest (SW) of CYMX. The pilot remained out of controlled airspace, then communicated with the tower by telephone before entering the area displaying the code 7600. This communication channel was interrupted due to poor quality. C-GIGW was finally able to establish radio communication with the tower and landed on Runway 29 safely at 1933Z. Operational impact: a Government of Canada, Department of National Defence McDonnell Douglas F-18 (VAPR01) from Montreal/Mirabel, QC (CYMX) to Montreal/Mirabel, QC (CYMX) broke off its approach to Runway 11 under the instructions of the Montreal terminal (relaying the requests from CYMX).

Incident at MONT-JOLI QC (CYYY) (Aerodrome, runway or taxiway shutdown)

At Mont-Joli, QC (CYYY), a vehicle (ST22) advised that Runway 06/24 and Runway 15/33 would be closed for less than an hour, starting at 1050Z. A voice NOTAM was issued. The Montreal area control centre (CZUL) was notified. At 1135Z, the airport manager (APM) transmitted a runway surface condition (RSC) /Canadian Runway Friction Index (CRFI), and Runway 06/24 was reopened. A NOTAM was issued by the APM for the closure of Runway 15/33. A Provincial Airlines de Havilland DHC-8-314 (C-GYCV/PVL3055) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Mont-Joli, QC (CYYY) had to enter into a holding pattern and their arrival was delayed by about 20 minutes.

A privately registered Mooney M20E from Montreal/St-Hubert, QC (CYHU) to Lachute, QC (CSE4) was observed at 1700 feet entering the airspace of Montreal/Mirabel, QC (CYMX), in the Southwest (SW) direction, without establishing communication with the air traffic service (ATS). The aircraft subsequently contacted the tower while already in the area. No loss of separation, no impact on operations.

Incident at CHIBOUGAMAU / CHAPAIS QC (CYMT)

The Joint Rescue Coordination Centre (JRCC) Trenton advised of an emergency locator transmitter (ELT) signal that was heard 40NM East Southeast (ESE) from Chibougamau/Chapais, QC (CYMT) for a privately registered Piper J3C-65S. An Air Creebec de Havilland DHC-8-101 (CRQ206) that was in the area conducted a search on 126.7 MHz, and did not receive the ELT signal. The flight service station (FSS) searched on 122.0 MHz, 121.5 MHz and 243.0 MHz, with no response. The FSS notified JRCC Trenton of the inconclusive results of the communication searches.

A fire alarm was heard in the Montreal Area Control Centre (ACC) building. The administrative staff present evacuated the building. In the operations room, only the evacuation pre-alarm was visible (strobe lights only). Sub-units stabilized their traffic and notified adjacent units. At 1824Z, the fire alarm and pre-alarm in the operations room ceased. Negligible impact. The Aviation Operations Centre (AVOPS) and the National Operations Centre (NOC) were notified.

Accident at 5119N / 07117W

The Joint Rescue Coordination Centre (JRCC) Trenton advised the area control centre (ACC) of a crash of a privately registered, amateur-built Odyssee at 180NM north of Bagotville, QC (CYBG). 2 souls on board, no injuries.
UPDATE: Aviation Incident Report: At 1708Z (1308 EDT), on September 19, 2020, a privately registered, amateur-built Odyssee aircraft crashed 180NM North of Bagotville, QC (CYBG). There were two (2) persons aboard, neither of whom were injured. A rescue team was on route to extract the individuals. The phase of flight at the time of the accident was not indicated. The Transportation Safety Board of Canada (TSB) was informed, as well as the Joint Rescue Coordination Centre (JRCC) Trenton.
UPDATE: TSB Report #A20Q0097: A privately operated, amateur-built Odyssey was on a local visual flight rules flight in the Savane River, QC, area, approximately 180NM north of Bagotville, QC (CYBG), with a passenger and a pilot on board. As the aircraft landed on the Savane River, upon contact with the water, the aircraft was pushed towards the shore by the current and ended its landing roll-out in the trees. There were no injuries and the aircraft sustained significant damage.

No assistance requested. The Joint Rescue Coordination Centre (JRCC) Trenton and the National Operations Centre (NOC) were advised. The aircraft landed without issue at 2255Z. Accident at 15NM NNW ST-FRÉDÉRIC QC (CSZ4)

Information was received from the Joint Rescue Coordination Center (JRCC) Trenton about a Grondair Cessna 172N (C-FDLJ) which had crashed at coordinate 4631.70N / 07110.60W. The extent of the damage is unknown. No injuries, no deaths. The National Operations Centre (NOC), the Aviation Operations Centre (AVOPS) and the Transportation Safety Board of Canada (TSB) were notified.
Update: TSB Report #A20Q0091: C-FDLJ, a Cessna 172N operated by Grondin Transports Inc., was on a local training flight from St-Frédéric, QC (CSZ4) airport with a student pilot alone on board. In a precautionary landing simulation practice exercise, the student pilot selected a field and then prepared the aircraft for an inspection at 500 feet AGL. The flaps were reportedly lowered to 20 degrees and power was reduced to a speed of about 70 knots (kts). After the inspection, the aircraft climbed to approximately 1000 feet AGL and the flaps were raised to 10 degrees. Towards the end of the left downwind leg, the student pilot observed a vertical speed of -1000 feet per minute (FPM) on the vertical speedometer (VSI) and the stall recovery technique was applied by pushing on the nose. The aircraft levelled off and VSI was observed at -1000 FPM again. The stall recovery manoeuvre was redone, but the situation remained the same. The manoeuvre was repeated a few times and the aircraft ended up very low. The student pilot saw 2 fields in front of them with a tree line separating them. The aircraft was turned right, and then left, in an attempt to reduce speed and allow a landing past the edge of the trees. The aircraft struck the edge of the trees in the left turn. About 1.5 meters of the left wing tore off as a result of the impact and caused the aircraft to turn more sharply. The portion of the left wing that was still attached to the aircraft touched down for a length of about 18 meters. The aircraft then inverted onto its back, the propeller struck the ground, followed by the engine. The aircraft bounced, then touched the ground, still on its back, slid for about 53 meters and then came to a stop about 137 meters from the edge of the trees. There was no fire. The pilot sustained serious injuries. The pilot evacuated themselves from the aircraft. Residents near the area came to the pilots aid and the pilot was taken to hospital.

Aviation Incident Report #16765: A Richcopter Inc. Cessna 150M (C-GUDB) landed and subsequently took off from the runway while a portion of it was closed by NOTAM (K2686/20). The pilot was warned by universal communications (UNICOM), but nevertheless arrived and took off from the threshold of Runway 05, which was closed. There was traffic present in the area at the time of the incident. There were personnel present on the runway at the time of the incident. No impact on operations. Accident at 3.2NM NW LES BERGERONNES QC (CTH3)

TSB Report #A20Q0082: C-GNZF, a Eurocopter AS350B2 operated by Canadian Helicopters Ltd, was on an external load flight between 2 transmission lines in the Sacré-Coeur, QC, area when the main rotor contacted and severed an electrical cable. The pilot felt vibrations and immediately deposited and released the external load on the ground. The aircraft has been positioned away from the load, where it has landed safely. The main rotor was damaged and there were no injuries.

Incident at Sherbrooke Street East, Montreal

Aviation Incident Report #16730: A Foxair Héliservice - Helico Pro Robinson R44 (C-FJLK) was transporting a client who had rented its services. At the request of the client (for recreational/personal reasons), C-FJLK landed in a small field near a condominium building on Sherbrooke Street East (in a residential area). Two calls from landing suits were made to 911 and police were dispatched to the scene. The pilot considered the landing safe as there were no electrical wires or other dangerous objects. Incident at In the vicinity of: LACHUTE QC (CSE4)

At 1413Z, a call came from an individual (a pilot) who reported having seen and heard a Cessna type aircraft (C150 or C172) that seemed to be having trouble with its engine and was heading south, either to Lachute, QC (CSE4) or Montreal/Mirabel, QC (CYMX). The person was approximately 38 km north of CSE4. The Area Control Centre (ACC) shift manager was notified. Around 1445Z, the ACC shift manager confirmed that an Aéroclub Gatineau Ottawa Inc. Cessna 150M (C-GEEH) from Ottawa/Gatineau, QC (CYND) to Mont-Joli, QC (CYYY) had an engine failure and landed without damage at CSE4.

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX) (Incursion - manoeuvring area, Communication error)

At Montreal/Mirabel, QC (CYMX), a vehicle (SV352+A+1) was towing an Airbus A343 (HFM564P) from the industrial terminal to the main terminal. The driver of SV352 communicated with the tower by telephone and was instructed to hold short of Taxiway H. This was due to a F18 that was taxiing on H. Once the F18 passed in front of them, SV652+A+1 taxied on Taxiways H and I without authorization or communication with the ground controller. Once on Taxiway I, the vehicle and controller were in contact. No impact.
