Incident at CORAL HARBOUR NU (CYZS) (ATM - operations)

The community aerodrome radio station (CARS) at CYZS was closed.

Incident at EDMONTON / U of A (STOLLERY CHILDREN'S HOSP) AB (Heli) (CEW7)

An intermittent emergency locator transmitter (ELT) signal started breaking squelch at 1812Z for about 5 seconds. At 1815Z the signal returned, strong, and steady for approximately 2 minutes. The Edmonton, AB (CYEG) terminal advised it was an inbound Shock Trauma Air Rescue Service Agusta AW139 (STAR3) on a flight to Edmonton/U of A (Stollery Childrens Hospital), AB (Heli) (CEW7). Information was passed to Edmonton (EG) centre Air Traffic Operations Specialist (ATOS). No impact to operations.

Incident at OXFORD HOUSE MB (CYOH) (Aerodrome - runway or taxiway surface condition, Aerodrome, runway or taxiway shutdown)

The runway at Oxford House, MB (CYOH) was closed due to an aircraft being stuck in a soft rut.

The pilot of a Goldcorp Canada Ltd. de Havilland DHC-8-314 (C-GMWT) from Opapimiskan Lake, ON (CKM8) to Thunder Bay, ON (CYQT) reported a possible bird strike on landing. Airport operations was advised and a runway inspection was performed. No bird found.

Incident at LANGLEY REGIONAL BC (CYNJ)

A privately registered Cessna 150F from Langley, BC (CYNJ) to Langley, BC (CYNJ) was cleared for a touch-and-go for Runway 01. The aircraft did not rotate for departure and continued off the hard surface into the grass. The aircraft taxied back onto the runway to exit at Taxiway B. No operational impact.

An Air Canada Airbus A320-211 (C-FGYL/ACA575) from Los Angeles Intl, CA (KLAX) to Vancouver Intl, BC (CYVR) declared a MAYDAY at 3 miles final Runway 08L due to a medical emergency.

A privately registered Diamond DA40 from Langley, BC (CYNJ) to Vancouver/Boundary Bay, BC (CZBB) taxied via Echo when it was cleared for Bravo to Runway 19. The tower informed the pilot of the routing error. No further impact to operations.

Incident at 6NM SW Niagara Falls, NY (KIAG)

An American, corporately registered Boeing 737-700 (N737CM) from Morristown, NJ (KMMU) to Pellston, MI (KPLN) transited through Toronto Centre, ON (CZY) airspace "no radio" (NORDO). N737CM failed to respond to the Cleveland Air Route Traffic Control Centre (ZOB) during the frequency change. Several attempts by ZOB and CZY to contact the N737CM were unsuccessful. N737CM was handed off to the Minneapolis Air Route Traffic Control Centre (ZMP) still NORDO. N737CM subsequently established contact with ZMP when they needed descent. No impact on operations.

A Jazz de Havilland DHC-8-402 (JZA8181) from Vancouver, BC (CYVR) to Fort St. John, BC (CYXJ) at FL200 picked up an emergency locator transmitter (ELT) signal in the vicinity of CYXJ. The source was unknown. No impact to operations.

Incident at KAMLOOPS BC (CYKA)

At 0043Z, A KF Cargo Convair 340 (C-GKFF/KFA578) departed Kamloops, BC (CYKA) for Vancouver Intl, BC (CYVR). At 0045Z the aircraft advised they had an open door indicator light and were returning to Kamloops. The aircraft landed without incident at 0049Z. No assistance required.

On departure from Montreal/St-Hubert, QC (CYHU), a Panorama Fixed Wings Pilatus PC-12/45 (PNO704) from Montreal/St-Hubert, QC (CYHU) to Northern Aroostook Regional, ME (KFVE) was flying a different flight plan than the one known by the air traffic controller (ATC). No impact.

Incident at ST. JOHN'S INTL NL (CYYT) (Aerodrome - other, Aerodrome, runway or taxiway shutdown)

A staff vehicle reported a small fire on the side of Runway 34 at approximately 1520Z. The runway was closed for approximately 15 minutes while fire crews responded. The cause of the fire was due to a flare used by wildlife patrol. A MEDEVAC aircraft had to change departure runways due to the closure, but there was no delay or impact on their departure.

Incident at RED DEER REGIONAL AB (CYQF)

A Sky Wings Aviation Academy Ltd. Cessna 172P (C-GJSK) from Red Deer Regional, AB (CYQF) to Red Deer Regional, AB (CYQF) advised they may be having radio issues while inbound over Pine Lake. When the aircraft was 5 miles to the west they switched their transponder to 7600. They were able to transmit but not receive communications. The aircraft landed at 1601Z. No impact to operations.

An American, corporately registered Boeing 737-436 (N734CK/KII918) from Milwaukee, WI (KMKE) to Winnipeg/James Armstrong Richardson, MB (CYWG) was cleared for the area navigation (RNAV) Z 18 approach and was subsequently observed to descend as low at 3100 feet where 3500 feet is required. No impact to operations.

An Air Canada Airbus A319-114 (ACA353) from Ottawa/MacDonald-Cartier, ON (CYOW) to Calgary, AB (CYYC) upon landing Runway 35R, reported a possible bird strike. Airport 1 conducted a runway inspection and found nothing. No impact to operations.

Incident at DRYDEN BEST WESTERN ON (Heli) (CKV3)

The pilot of a Provincial Helicopters Ltd. Bell 206L1-C30P (C-GPHV) on a flight to Dryden Best Western, ON (Heli) (CKV3) reported on final at 2240Z, but no down call was received. Dryden Best Western was contacted via telephone and they confirmed visual with helicopter at the helipad. No impact to operations.

Incident at PITT MEADOWS BC (CYPK)

After landing on Runway 26L, a privately registered Murphy Maverick ultralight on a flight from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK) exited the runway into the grass South of the runway and East of Taxiway B and subsequently ground-looped after re-entering Taxiway B. The aircraft was able to taxi to Apron 2 without assistance. No impact on operations.

A KF Cargo Convair 340 (C-GKFF/KFA578) from Kamloops, BC (CYKA) to Kamloops, BC (CYKA) returned shortly after departure due to a mechanical issue. The aircraft landed safely. No impact to operations.

Incident at PITT MEADOWS BC (CYPK)

After landing on Runway 26L, a Pacific Flying Club Cessna 172P (C-GBLP) from Vancouver/Boundary Bay, BC (CZBB) to Pitt Meadows, BC (CYPK) was instructed to exit on Taxiway C after passing Taxiway B. Instead, the aircraft stopped and backtracked to exit on Taxiway B. As a result, a Pacific Rim Aviation Academy Diamond DA 20-C1 (C-GGUK) from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK) on final was instructed to pull up and go around. Additionally, C-GBLP attempted to taxi to Apron 1 without contacting Ground before being stopped by air traffic control (ATC).

Incident at Kelowna

A privately registered Cessna U206 from Edmonton/Villeneuve, AB (CZVL) to Kelowna, BC (CYLW) entered transponder only airspace without a functioning transponder. The aircraft landed safely.

Incident at NANAIMO AIRPORT

Upon landing Runway 16, a WestJet Encore de Havilland DHC-8-402 (C-GPWE/WEN3109) from Calgary, AB (CYYC) to Nanaimo, BC (CYCD) struck at least one small bird. Runway foreign object debris check completed.

Incident at PENTICTON BC (CYYF) (Other operational incident, Aerodrome, runway or taxiway shutdown)

Due to a forest fire Northwest of Penticton, BC (CYYF). The Canadian Aviation Regulation (CARs) 601.15 implementation restricted Penticton, BC (CYYF) operations, with no arrivals or departures. Several known departures were delayed or cancelled. 3 known arrivals were delayed or diverted. During the review, the restriction was reduced from 5 nautical miles to 2 nautical miles, and operations resumed at Penticton with less restrictions.

Incident at PENTICTON BC (CYYF)

A privately registered Beech M35 from Osoyoos, BC (CBB9) to Penticton, BC (CYYF) received an for landing Runway 34. On its short final, the aircraft became "no radio" (NORDO). The aircraft landed and taxied safely. No operational impact.

A loud and clear emergency locator transmitter (ELT) signal was heard by various aircraft in the triangle of Trois-Rivières (CYRQ) - Quebec/Jean Lesage Intl (CYQB) - Sherbrooke (CYSC). Signal ended at 1200Z. The source was identified as the accidental activation by a Richcopter Inc. Cessna 150M (C-FTOD) on the ground at Drummondville, QC (CSC3). The Joint Rescue Coordination Centre (JRCC) Trenton was advised.

A North Cariboo Flying Service British Aerospace Avro 146 series RJ100 (NCB1503) from Terrace, BC (CYXT) to Calgary Intl, AB (CYYC) advised they had a cracked windshield and continued their flight at lower altitude to keep the pressure down. No further assistance required. The aircraft landed without incident 0214Z. No impact to operations.

The pilot of a Top Aces Inc. Learjet 35A (C-GTXE/BRUSR02) on a flight from Thunder Bay, ON (CYQT) reported their oil engine pressure dropping and that they were diverting to Winnipeg/James Armstrong Richardson, MB (CYWG). BRUSR02 did not declare an emergency and dumped its fuel at 9000ft for approximately 5 minutes 1NM Northwest of CYWG. BRUSR02 landed without incident. No impact to operations.

An Aerovias de Mexico Boeing 737-800 (XAAMB/AMX696) from Mexico City, Mexico (MMMX) to Vancouver Intl, BC (CYVR) was observed descending without air traffic control (ATC) authorization. No impact on operations.

Incident at OKANAGAN (PENTICTON) BC (ON) NDB

A WestJet Encore de Havilland DHC-8-402 (C-FOEN/WEN3281) from Calgary, AB (CYYC) to Penticton, BC (CYYF) was observed making a left turn after the Okanagan (Penticton), BC (ON) NDB without coordination with air traffic control (ATC). This turn left the aircraft at an altitude lower than the Minimum Radar Vectoring Altitude (MRVA). When questioned, WEN3281 indicated they were completing a company hold. Later, the crew confirmed they did not receive a hold clearance. No impact on operations.
