A Mountain Aviation Cessna 750 (FTH517) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Cleveland Intl, OH (KCLE) deviated from the standard instrument departure (SID) after departing Runway 23. Controller corrected heading. No impact to operations.

A Sky Regional Airlines Embraer ERJ 170-200SU (C-FEKH/SKV7607) on a flight from Boston Intl, MA (KBOS) to Toronto/Lester B. Pearson Intl, ON (CYYZ) on final approach for Runway 24R, initiated a missed approach due to previous landers reporting poor braking condition. SKV7607 landed Runway 23 at 0120Z without incident. No impact to operations.

A Coastal Pacific Aviation Cessna 172M C-GUKI from CYXX to CYXX was the next departure, and was instructed to abort their takeoff by air traffic control (ATC).

Incident at 6320N / 08841W

A Summit Air BAE Systems RJ85 (C-FLRJ/operating as First Air flight FAB239) was on a flight from Iqaluit, NU (CYFB) to Rankin Inlet, NU (CYRT). The pilot reported an issue with their flaps and was diverted to Yellowknife, NT (CYZF). The aircraft advised the tower that they would be landing flapless, and requested airport emergency services be on standby on arrival as a precaution. The aircraft landed without incident at 0056Z. No impact to operations. (Duplicate AOR 258479-V1 deleted).

A United States Air Force Boeing KC-135R (JEEP41) on a flight from Harrison Township, MI (KMTC) reported an unspecified mechanical issue and requested to return to KMTC. No emergency declared. No impact to operations.

Incident at In the vicinity of: MASSET BC (Water) (CBN4)

Aviation Incident Report# 16293: An Inland Air Charters de Havilland DHC-2 (C-GCYM) from Prince Rupert/Seal Cove Water Aerodrome, BC (CZSW) to Masset Water Aerodrome, BC (CBN4) was leaving the Masset area to the east level at 1000ft. A Rocky View Air Beech 390 (C-GYPV) was sighted on the right hand side of aircraft 500ft away. C-GYPV had not made any radio calls on 122.7 Masset, and was not flying a proper IFR inbound approach. There was a risk of mid air collision.

Incident at 2NM E VANCOUVER INTL BC (CYVR)

A WestJet Boeing 737-7CT (C-GWSU/WJA448) on a flight from Vancouver Intl, BC (CYVR) to Winnipeg/James Armstrong Richardson Intl, MB (CYWG) received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) while climbing through 1400ft. Only observed radar traffic was 600ft below them. No other aircraft were affected.

A Jazz de Havilland DHC-8-402 (C-GKUK/JZA8215) on a flight from Vancouver Intl, BC (CYVR) to Cranbrook/Canadian Rockies Intl, BC (CYXC) reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and levelled off at 2000ft. There were no other aircraft visible in the vicinity of JZA8215. No other aircraft were affected.

A WestJet Encore de Havilland DHC-8-402 (WEN3524) from Toronto/Lester B. Pearson, ON (CYYZ) to Montreal/Trudeau, QC (CYUL) declared PAN PAN with smoke in the cockpit. Emergency declared. Given priority vectors for Runway 24L at CYUL. Landed safely at 1716Z. Impact: several aircraft delayed on arrival for approximately 10 minutes each.

New York Area Control Centre (ACC) advised that a Turkish Airlines Airbus A330-200 (TCJND/THY183) on a flight from Istanbul, Turkey (LTFM) to Havana, Cuba (MUHA) in the vicinity of 3800N04900W was diverting to St. Johns Intl, NL (CYYT) due to a medical emergency. The flight landed at 0851Z . No impact on operations.

An Air France Boeing 777-300 (FGSQO/AFR760) on a flight from (MDSD) to Paris/Charles de Gaulle, France (LFPG) declared a medical emergency at 4903N04008W and requested to divert to St. Johns Intl, NL (CYYT). The flight was re-cleared as requested and landed at 0724Z. No impact on operations.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY) (Aerodrome, runway or taxiway shutdown, Aerodrome - runway or taxiway surface condition)

An Alkan Air Beech 200 (C-FAKN/AKN216) on a flight from Mayo, YT (CYMA) to Whitehorse/Erik Nielsen Intl, YT (CYXY) landed Runway 14R at 0010Z and could not make the Runway 19 exit due to two flat tires. Multiple aircraft subsequently denied clearance as Runway 14R was blocked by AKN216, and Runway 14L was unserviceable due to three inches of snow cover.

Incident at EDMONTON INTL AB (CYEG)

A Saskatchewan Air Ambulance Beech 200 (SLG4) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Edmonton Intl, AB (CYEG) was assigned the IGSOX standard terminal arrival (STAR) for Runway 12 but flew the STAR for Runway 30. Controller intervened and aircraft was redirected for Runway 12. No impact to the operation.

Incident at SASKATOON / JOHN G. DIEFENBAKER INTL SK (CYXE)

A Jazz de Havilland DHC-8-402 (C-GGMN/JZA8591) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Calgary Intl, AB (CYYC) was given and read back a clearance to FL240. The controller observed the pilot climbing through 24,300ft and queried the pilot. Pilot reached 24,900ft before returning to 24,000ft. No impact to operations.

Incident at SASKATOON / JOHN G. DIEFENBAKER INTL SK (CYXE)

A WestJet Encore de Havilland DHC-8-402 (C-GENU/WEN3371) on a flight from Saskatoon/John G. Diefenbaker Intl, SK (CYXE) to Calgary Intl, AB (CYYC) hit a coyote with its left main gear while departing Runway 27 at 0334Z. The aircraft landed safely on Runway 15 at 0347Z. No impact to operations.

The community aerodrome radio station (CARS) operator at Taloyoak, NU (CYYH) reported that a Canadian North de Havilland DHC-8-106 (C-GRGO/MPE8656) on a flight to Taloyoak, NU (CYYH) failed to contact airport radio on the mandatory frequency (MF) until less than 1 minute back on final Runway 33T and did not provide departure (DEP) aerodrome, distance from CYYH, or an estimated time of arrival (ETA).

An Arrowhead Helicopters Bell 407 (C-FARE) inbound to Stony Rapids, SK (CYSF) from SE (target observed on radar) failed to contact Regina Intl, SK (CYQR) Flight Service Station (FSS) prior to entering the mandatory frequency (MF) area. FSS attempted contact with no response. First contact was on short final for apron. The pilot indicated that their global positioning system (GPS) indicated frequency 122.2 instead of 122.1. No impact on operations.

An American corporately registered Cessna 750 (N688LB) on a flight from Miami-Opa Locka, FL (KOPF) to Toronto/Lester B. Pearson Intl, ON (CYYZ) was instructed to initiate a missed approach to Runway 23 due to an Air France Airbus A350-900 (AFR351) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Paris/Charles de Gaulle Intl, France (LFPG) on the runway slow to depart. N688LB landed Runway 23 at 0124Z without incident. No impact to operations.

A Porter Airlines de Havilland DHC-8-402 (POE229) on a flight from Toronto/Billy Bishop, ON (CYTZ) to Ottawa/MacDonald-Cartier Intl, ON (CYOW) was given the overshoot instructions at 2 miles final as previous lander missed Foxtrot Taxiway exit and had to exit at the end of the runway. Runway was snow covered and aircraft took a long to turn off runway. POE229 landed at 1831Z.

An Air Transat Airbus A310 (TSC773) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Vancouver, BC (CYVR) returned and landed because its gear did not retract after takeoff. The aircraft landed without incident at 0117Z.

Air traffic services (ATS) intervened so that the required separation would be maintained.

Incident at BIRLI intersection

On departure, a Sky Regional Airlines Embraer ERJ 170-200SU (SKV7723) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Houston Intl, TX (KIAH) was cleared to climb to 15 000 feet. A Delta Air Lines McDonnell Douglas MD 88 (DAL2259) on a flight from Atlanta Intl, GA (KATL) to Toronto/Lester B. Pearson Intl, ON (CYYZ) was inbound from the south at 10 000 feet. SKV7723 was turned to heading of 150 degrees and instructed to stop the climb at 9000 feet. There was no loss of separation and no impact to operations.

Incident at CALGARY INTL AB (CYYC)

A Compass Airlines Embraer ERJ 170-200 LR (N620CZ/CPZ5773) on a flight from Calgary Intl, AB (CYYC) to Salt Lake City Intl, UT (KSLC) was observed on radar not making the required standard instrument departure (SID) turn to heading 360 after departing Runway 35R.

Incident at IQALUIT NU (CYFB)

A strong emergency locator transmitter (ELT) signal was heard by the flight service station (FSS) on 121.5 intermittently from 1626Z to 1630Z. A First Air Aerospatiale ATR 42-500 (C-FTIL/FAB874) from Iqaluit, NU (CYFB) to Igloolik, NU (CYGT) on apron advised that it was their ELT and they will attempt to shut it off. Montreal Area Control Centre (CZUL) shift manager advised. No impact on operations.

Incident at YORKTON MUNI SK (CYQV)

While a vehicle was authorized to be on Yorkton, SK (CYQV) Runway 04/22 with permission to cross Runway 13/31 until further advised, a Leading Edge Aviation Ltd. Cessna 172M (C-GJKR) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) departed Runway 31 without first contacting the flight service station (FSS) for a departure advisory. C-GJKR had taxied to the apron after doing circuits in order to drop someone off before resuming circuits.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

A Cargojet Airways Boeing 767-300 (CJT573) from Hamilton, ON (CYHM) to Winnipeg/Richardson, MB (CYWG) conducted a missed approach for Runway 36 due to an unsafe gear indication. Aircraft returned to Arrival for vectors and troubleshooting. Landed Runway 36 safely at 1008Z. No operational impact.

Pearson, ON (CYYZ) to Philadelphia, PA (KPHL) that was not clear of the runway. ACA607 landed at 1508Z. No operational impact.

B Pearson, ON (CYYZ) to Los Angeles, CA (KLAX) crossed the hold line for Taxiway Q and infringed on the Obstacle Limitation Surface (OLS) requiring to instruct a Philippine Airlines Airbus A350-900 (RPC3507/PAL118) to initiate a missed approach for Runway 23. PAL118 landed Runway 2342Z without incident. No impact to operations and Transportation Safety Board (TSB) notified.

Incident at VICTORIA INTL BC (CYYJ)

A Horizon Air Industries de Havilland DHC-8-400 (QXE2384) from Seattle-Tacoma, WA (KSEA) to Victoria, BC (CYYJ) landed on Runway 09 and reported a possible bird strike. The Aircraft Rescue and fire Fighting (ARFF) unit inspected the runway and did not find anything. No operational impact.
