Incident at TORONTO / OSHAWA EXECUTIVE AIRPORT ON (CYOO)

A 2169240 Ontario Inc. Cessna T182T (C-GDKP) from Orillia Rama, ON (CNJ4) to Toronto/Oshawa, ON (CYOO) reported a bird strike approximately 0.5 miles final for Runway 23. The pilot reported that it was a propeller strike and that no mechanical or structural issues were observed. The aircraft landed without incident and taxied to the apron without issue.

Incident at PITT MEADOWS BC (CYPK) (Dangerous goods/hazardous materials)

At Pitt Meadows, BC (CYPK), a pilot reported a fuel leakage from a Canadian Flight Centre Cessna 152 (C-GMZW) that was parked on the apron. The fire department was called to clean up the spilled fuel. The tower manager and airport manager were notified. No impact to operations.

A Transwest Air Aerospatiale ATR 42-300 (C-GWWD) from Saskatoon/John G. Diefenbaker, SK (CYXE) to La Ronge (Barber Field), SK (CYVC) rejected take off at 1252Z due to a coyote on the runway. The aircraft departed safely at 1255Z.

Pearson, ON (CYYZ) was instructed to conduct a go-around for Runway 24L due to a Jazz Embraer ERJ 170-200 SU (C-FEJY/JZA7609) from Toronto/Lester B. Pearson, ON (CYYZ) to Chicago/OHare, IL (KORD) crossing the Runway 24L approach hold line. ASP519 landed on Runway 24L without issue at 1852Z. No other impact to operations.

Incident at ST. CATHARINES / NIAGARA DISTRICT ON (CYSN)

A privately registered Piper PA-24-250 from Ottawa/Carp, ON (CYRP) to St. Catharines/Niagara, ON (CYSN) experienced an issue with the brakes shortly after landing on Runway 24 and ended up on the grass North of the runway. The aircraft taxied back to the airport without assistance. No impact to operations.

A Thunder Airlines Beech 100 (THU101) from Timmins/Victor M. Power, ON (CYTS) to Fort Albany, ON (CYFA) advised Air Traffic Control (ATC) of vehicles on the runway at Kashechewan, ON (CZKE). The shift manager called the airport manager to confirm construction on the runway and a NOTAM was required. NOTAM issued from 1900Z to 2100Z.

A North Bay Flying Club Cessna 182Q (C-GHGJ) from North Bay, ON (CYYB) to North Bay, ON (CYYB) departed from Runway 26 while two vehicles (ST42 and TK80) were at the end of the runway near Taxiway J.

At Ottawa/Gatineau, QC (CYND), 2 skydivers landed in the runway protected area (RPA) of Runway 27. Three aircraft landed while the skydivers were waiting for a vehicle (ST22) to pick them up. The landing aircraft included a privately registered Piper PA-28-180 from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), a Parachutisme Adrenaline Cessna 208B (C-GYRQ) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), and an Academie de Pilotage Internationale Cessna 172N (C-GYTI) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND).

Incident at ST-JEAN QC (CYJN) (FOD (foreign object debris), Aerodrome - runway or taxiway surface condition)

A Dorval Aviation Cessna 172M (C-FCEI) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Montreal/Pierre Elliott Trudeau, QC (CYUL) reported an object on Runway 29, near the threshold of Runway 11, at St-Jean, QC (CYJN). A vehicle was sent to pick up the item. The aircraft conducted a circuit at 1100ft while waiting for the object to be picked up. No other impact.

Accident at BAIE-COMEAU QC (CYBC) (Aerodrome, runway or taxiway shutdown)

A privately registered Cessna 182L on a flight to Baie-Comeau, QC (CYBC) crashed at the Runway 10 threshold. Two passengers suffered minor injuries. Impact: The runway was closed and the localizer (LOC) was unavailable. An Air Inuit (AIE850) de Havilland DHC-8-300 from Québec/Jean-Lesage, QC (CYQB) to Baie-Comeau, QC (CYBC) changed its destination to Havre-Saint-Pierre, QC (CYGV).

Incident at 6346N / 11823W

A Buffalo Airways Beech 100 (BFL102) from Déline, NT (CYWJ) to Yellowknife, NT (CYZF) was observed level at FL190 without a clearance. At 6322N 11659W (about 15 minutes later), the aircraft was observed descending to FL170 prior to making contact with air traffic control (ATC).

An Aries Aviation Services Corp. Piper PA-31 (C-FVZM) from Calgary/Springbank, AB (CYBW) to Pitt Meadows, BC (CYPK) was instructed to climb to 14 000 feet. At 1605Z, air traffic control (ATC) observed the aircraft climbing to 14 600 feet. C-FVZM was instructed to descend to 14 000 feet. No impact to the operation.

Incident at 76NM REGINA INTL SK VOR/DME (YQR)

A Super T Aviation Beech B200 (C-GPIA) from Regina, SK (CYQR) to Sioux Lookout, ON (CYXL) was observed to be going off course and, when queried, the pilot stated there was an issue with the flight management system (FMS). The aircraft was vectored back to CYQR and, approximately 60NM from the aerodrome, the pilot reported the FMS issue was resolved and requested clearance to CYXL. No impact to operations.

Incident at NUBER intersection

An Air Canada Boeing 787-8 (ACA7284) from Tokyo/Narita, Japan (RJAA) to Toronto/Lester B. Pearson, ON (CYYZ), flying the NUBER4 standard terminal arrival (STAR), was experiencing flight management system (FMS) discrepancies between the FMS path and the air traffic control (ATC) path. The aircraft was 2.2NM off NUBER and NUBER direct PENGO without ROKTO. No operational impact.

Accident at 20NM N CAMPBELL RIVER BC (CYBL)

At 1522Z, the Campbell River flight service station (FSS) advised that, at 1445Z, a CBE Construction Ltd. de Havilland DHC-2 MK. III (C-FOES) from Port Hardy, BC (CYZT) to Campbell River, BC (CYBL) crashed into the water, approximately 20NM north of CYBL, under unknown circumstances. No operational impact.

A Cargojet Airways Ltd. Boeing 767-323 (C-FCCJ/CJT916) from Hamilton, ON (CYHM) to Cincinnati/Northern Kentucky, KY (KCVG) returned to land at CYHM due to a cabin pressure problem. No emergency declared or equipment required. No impact on operations.

Incident at Kelowna

At Kelowna, BC (CYLW), an emergency locator transmitter (ELT) was activated at 1835Z. The Joint Rescue Coordination Centre (JRCC) Victoria called up and identified the aircraft as a privately registered American Aviation AA-5A. A staff vehicle located the aircraft at the Stolaris hangar, where it was being worked on.

Incident at Kelowna

An American, corporately registered Cessna 208 (N208JK) from Coeur dAlene, ID (KCOE) to Kelowna, BC (CYLW) entered the CYLW control zone without establishing contact and without being on a VFR route. The area control centre (ACC) had reached out when the aircraft was 15 miles out to advise that they were no longer in contact. Several attempts at contact were made and the aircraft finally called up when 1.5NM into the zone. No operational impact.
